Detection of the Experession and Significant of CK19 and EMA in the Pelvic Lymoh Nodes with Ⅰ~Ⅱ Stage Squamous Cervical Carcinoma

摘要 目的:研究CK19(cytokeratin19)和EMA(epithelial membrane antigen)在Ⅰ~Ⅱ期宫颈鳞癌盆腔淋巴结中的表达,探讨与宫颈鳞癌盆腔淋巴结微小转移的相关性。方法:采用免疫组化法检测76例Ⅰ~Ⅱ期宫颈鳞癌患者盆腔淋巴结中CK19和EMA的表达。结果:①免疫组化结果显示:11例经常规HE病理检测阳性的盆腔淋巴结13枚,皆呈强阳性表达,CK19、EMA阳性表达率均为100%。65例经常规HE病理检测阴性的盆腔淋巴结中CK19呈阳性表达23例,阳性淋巴结33枚(33/241),阳性表达率13.7%,EMA呈阳性表达21例,阳性淋巴结31(31/241)枚,阳性表达率为12.9%,两组差异显著(P<0.05)。②宫颈鳞癌盆腔淋巴结微转移检出率与年龄、FIGO分期、病理分级无关(P>0.05),与有无淋巴结转移有关(P>0.05)。结论:①在淋巴结中CK19、EMA表达,可以作为Ⅰ~Ⅱ期宫颈鳞癌盆腔淋巴结微转移的生物学指标,提示可能存在宫颈鳞癌淋巴结微转移。 Objective: To study the expressions of CK19 (cytokeratin19) and EMA (epithelial membrane antigen) in the pelvic lymph nodes with Ⅰ~Ⅱ stage squamous cervical carcinoma, and their relations to the micrometastases in the pelvic lymph nodes with squamous cervical carcinoma. Methods: To use immunohistochmical method to detect the expressions of CK19 and EMA in the pelvic lymph nodes from 76patients with Ⅰ~Ⅱ stage squamous cervical carcinoma. Results : ① It is indicated Dy immunohistoehmical results that In 13 positive lymph nodes detected by normal HE from 11 cases, the expressions of both CK19 and EMA are all strong positive with 100 % positive rate. And meanwhile , in 241 negative lymph nodes detected by normal HE from 65 cases the expressions of CK19 in 33 lymph nodes from 23 cases are positive with 13.7 % positive rate (33/241) and the expressions of EMA in 31 nodes from 21 cases are positive with 12.9 % positive rate (31/241). There is a significant difference between two groups(P 〈0.05).②The detection rate of the micrometastases in the pelvic lymph nodes with cervical cancers has nothing to with the patients' age, FIGO stages (P 〉0.05), but it has something to do with the metastases in the lymph nodes (P 〉 0.05). Conclusion: The expressions of CK19 and EMA can be taken as a biology targent on the micro - metastases in the pelvic lymph nodes withⅠ~Ⅱ stage squamous cervical carcinoma to predict there may be the micrometastases in the pelvic lymph nodes with squamous cervical carcinoma.
