川芎嗪对硝酸甘油耐药的拮抗作用 被引量:10

摘要 采用随机、对照、单盲法观察川芎嗪对硝酸甘油耐药的拮抗作用。 60例不稳定型心绞痛患者随机分为空白对照组、血管紧张素转换酶抑制剂 (ACEI)组及川芎嗪组。 3组均以硝酸甘油 (NTG)持续静脉滴注 2 4h作为基础治疗 ,ACEI组服开搏通 1 2 .5mg,2次 ,川芎嗪组静滴川芎嗪 1 .6mg/kg,2次 ,观察临床症状、收缩压变化并测定血浆一氧化氮 (NO)、脂质过氧化物 (LPO)、超氧化物歧化酶 (SOD)、内皮素(ET)、血管紧张素Ⅱ (AngⅡ )的水平。结果显示 ,在NTG静滴过程中发生了耐药现象 ;川芎嗪与开搏通均有抗耐药作用 ,两组分别与空白对照组比较 ,均有显著性差异 (P <0 .0 5) ;川芎嗪组与ACEI组比较 ,无显著性差异 (P >0 .0 5)。川芎嗪可能通过对抗氧自由基而发挥其拮抗效应。 Objective:To investigate the effects and the mechanism of ligustrazine on nitroglycerin (NTG) resistance.Method:60 cases of unstable angina patients were allocated to three groups:NTG group(Group Ⅰ),TNG plus captopril group (Group Ⅱ),and NTG plus ligustrazine group(Group Ⅲ).NTG was administrated by intravenous drip for 24 hours,captopril 12.5 mg was administrated orally and ligustrazine 1.6mg/kg by intravenous drip.The percentage of NTG resistance,systolic blood pressure(SBP),and the level of plasma lipid peroxide (LPO),surperoxide dismutase(SOD),nitric oxide (NO),endothelin(ET) and angiotension Ⅱ(Agn Ⅱ)were investigated.Results:(1)the percentage of NTG resistance in Group Ⅱ and Group Ⅲ were lower than that in Group Ⅰ(P<0.05);(2)plasma nitric oxide serves as an indicator of NTG resistance;(3)Ligustrazine and captopril exert similar effects in counteracting NTG resistance.Conclusion:Inhibition of oxygen free radicals system may be the mechanism of ligustrazine in counteracting NTG resistance.
