心脏再同步化治疗对失同步化缺血性心力衰竭心肌内向整流钾电流的影响 被引量:5

摘要 目的:研究失同步化缺血性心力衰竭心肌内向整流钾电流(IK1)的变化及其心脏再同步化治疗(CRT)的作用。方法:取西北犬行左束支射频消融术和冠状动脉前降支结扎术建立失同步化缺血性心力衰竭模型(n=14)。行CRT治疗(n=7)后分离左室侧壁和室间隔心肌细胞,用全细胞膜片钳法检测IK1,同期测定血流动力学和超声心动图学指标。结果:失同步化组的IK1密度较对照组降低[室间隔:(0.70±0.31)pA/pFvs(1.60±0.28)pA/pF,P<0.05;左室侧壁:(1.20±0.34)pA/pFvs(1.75±0.31)pA/pF,P<0.05],且室间隔心肌细胞的IK1低于侧壁[(0.70±0.31)pA/pFvs(1.20±0.34)pA/pF,P<0.05],CRT升高了失同步化心力衰竭犬的室间隔和左室侧壁心肌的IK1[(1.50±0.30)pA/pFvs(0.70±0.31)pA/pF,P<0.05;(1.65±0.39)pA/pFvs(1.20±0.34)pA/pF,P<0.05),减小了室间隔和左室侧壁心肌IK1的差别[(1.50±0.30)pA/pFvs(1.65±0.39)pA/pF,P>0.05]。结论:CRT部分地逆转失同步化缺血性心力衰竭的IK1重构,减低IK1的各向异性。
