痔上黏膜环切钉合术+直肠前壁修补术+直肠黏膜柱状缝合治疗老年出口梗阻型便秘的前瞻性研究被引量：6

摘要目的:探讨老年出口梗阻型便秘患者接受痔上黏膜环切钉合术(procedure for prolapse and hemorrhoids stapling operation,PPH)+直肠前壁修补术+直肠黏膜柱状缝合术治疗的形态学及功能学改善程度。方法:前瞻性选取2008年3月至2010年12月间华中科技大学同济医学院附属荆州医院肛肠外科行该手术患者83例,分别于术前、术后3个月、术后6个月行排粪造影检查,并采用Longo出口梗阻评分及Wexner便秘严重程度评分评估便秘症状。结果:术后3、6个月患者的直肠前突、黏膜内脱垂、黏膜内套叠较术前明显下降(P<0.05),Longo出口梗阻型便秘评分由术前的(17.0±0.3)分下降至术后的(7.7±0.5)分,Wexner便秘严重程度评分由术前的(16.4±4.3)分下降至术后的(6.1±0.1)分(P<0.05)。结论:PPH+直肠前壁修补术+直肠黏膜柱状缝合术能有效纠正出口梗阻型便秘的形态及功能紊乱,从而改善便秘症状。
