缓解期精神分裂症患者心身症状与自动思维

The Automatic Thoughts and Psychosomatic Symptom of Patients with Schizophrenia

目的探讨缓解期精神分裂症患者自动思维特点及其与心身症状的相关性。方法采用自动思维问卷及心身症状评定量表对152例缓解期精神分裂症患者进行评定,并进行统计分析。结果①缓解期精神分裂症患者症状自评量表(SCL-90)评分结果与常模比较,除躯体化、人际敏感及敌对因子与常模比较无显著差异,其余因子均有显著差异(t=2.145～7.852;P<0.05);SCL-90因子得分轻度异常与明显异常者检验结果显示:各因子均存在显著差异(t=-6.361～-11.603;P<0.001);②缓解期精神分裂症患者的负性自动思维得分显著高于正常水平,男女被试自动思维得分无统计学差异;③自动思维30个条目与SCL-90总分存在显著相关性(r=0.226～0.723,P<0.01);④以心身症状总分为因变量,自动思维条目(N1-N30)为自变量进行多元逐步回归分析,结果发现依次进入方程的是:"为什么我总不能成功;我让人失望;不可能干好;有些东西必须改变;我肯定有问题;我无法坚持下去;我觉得孤立无援;我不好";⑤自动思维与SCL-90总分及各因子的一元线性回归分析提示:自动思维总分对各因子及总分具有显著预测作用。结论缓解期精神分裂症患者存在明显的心身症状,自动思维对缓解期精神分裂症的心身症状具有重要预测作用。

Objective To analyze the relationship between automatic thoughts and psychosomatic symptom of remission patients with schizophrenia. Methods A total of 152 patients with schizophrenia were evaluated by automatic thoughts questionnaire and SCL--90 scale,all data was analyzed by spssl7.0（P（0.05）. Results ①The SCL--90 scores were more lower than the norm ex- cept of somatization,interpersona[ sensitivity and hostility （t = 2. 145 ～7. 852, P〈0.05）; There were significant differences in the scores of SCL--90（t=-6. 361～-11. 603,P=0. 000）between the normal and the abnormal group② The automatic thoughts was much more than the norm ,and there was no gender difference;③ 30 items of automatic thoughts were significantly related to the total score of SCL--902④ The total score of SCL--90 as dependent variable and the items of automatic thoughts as independent variable, item 3,5,24,26,27,12,25 and 2 entered the regression equation by turn ⑤A linear regression analysis showed automatic thoughts could better predict of each factor and total score of SCL-- 90. Conclusion Remission patients with schizophrenia have obvious psychosomatic symptoms and automatic thoughts, automatic thoughts can better predict the symptoms of schizophrenia psychosomatic.
