肺痿冲剂方治疗肺间质纤维化61例疗效观察

A Clinical Observation on Feiwei Granules for 61 Cases of Pulmonary Fibrosis

目的观察肺痿冲剂方治疗肺间质纤维化(IPF)的临床疗效。方法将120例IPF患者随机分为治疗组61例、对照组59例,两组患者均进行基础治疗,治疗组同时给予肺痿冲剂方每次100ml,每日2次口服;对照组给予醋酸泼尼松片每次30mg,每日1次口服。治疗2个月后评价两组患者中医证候疗效及患者满意度,并观察两组患者治疗前后中医证候评分、呼吸困难评分、生活质量评分、6分钟步行试验、肺功能[包括肺总量(TLC)、用力肺活量(FVC)、一秒用力呼气容积(FEV1)、FEV1/FVC、一氧化碳弥散量(DLCO)]。结果治疗组与对照组中医证候总有效率分别为62.3%、32.2%,两组比较差异有统计学意义(P<0.01)。治疗组与对照组患者总满意率分别为91.4%、33.3%,两组比较差异有统计学意义(P<0.01)。治疗后治疗组中医证候评分、呼吸困难评分、生活质量评分、6分钟步行试验、肺功能TLC、FEV1均较治疗前改善(P<0.05或P<0.01);对照组中医证候部分指标评分、呼吸困难评分、生活质量部分指标评分较治疗前改善(P<0.05或P<0.01),但肺功能FVC反而较治疗前恶化(P<0.05)。治疗后治疗组与对照组比较,中医证候部分指标评分、生活质量部分指标评分、6分钟步行试验、肺功能FVC差异有统计学意义(P<0.05或P<0.01),治疗组疗效优于对照组。结论肺痿冲剂方治疗IPF可减轻患者中医、呼吸困难症状,改善患者生活质量、肺功能,且患者满意度高。

Objective To observe the therapeutic effect of Feiwei Granules （granules for lung wilting） for pulmonary fibro- sis. Methods Totally 120 cases of pulmonary fibrosis were randomized into the treatment group and control group, with 6] and 59 in each. Both groups were given the fundamental treatment. The treatment group was given 100ml Feiwei Granules twice daily. The control group was given 30 mg prednisone once daily. The treatment lasted for 2 months. The Chinese medi- cine syndrome efficacy and patient satisfaction was evaluated. The Chinese medicine syndrome score, dyspnea score, quality of life score （QOLS）, 6-minute walk test, total lung capacity （TLC）, forced vital capacity （FVC）, forced expiratory volume in one second （FEV1）, FEV1/FVC and diffusing capacity of the lung for carbon monoxide （DLCO） were observed before and af- ter treatment. Results The Chinese medicine syndrome efficacy in the treatment group and control group was 62.3% and 32. 2% respectively, with a significant difference between groups （P〈0. 01）. The patient satisfaction rate in the treatment group and control group was 91.4% and 33.3% respectively, with a significant difference between groups （P〈0. 01）. The Chinese medicine syndrome score, dyspnea score, QOLS, 6-minute walk test, TLC and FEV1 were significantly improved in the treat- ment group after treatment （P〈0.05 or P〈0.01）. The Chinese medicine syndrome score, dyspnea score and QOLS were sig- nificantly improved in the control group after treatment （P〈0.05 or P〈0.01）, but FVC was deteriorated after treatment （P〈0.05）. The treatment group was more effective in Chinese medicine syndrome score, QOLS, 6-minute walk test and FVC as compared with the control group （P〈0.05 or P〈0.01）. Conclusion Feiwei Granules is effective for pulmonary fibrosis in relieving Chinese medicine symptoms and dyspnea and improving QOL and lung function.
