窒息新生儿尿肾脏损伤因子-1检测在早期诊断急性肾损伤中的价值 被引量:6

摘要 目的探讨尿肾脏损伤因子-1(KIM-1)检测在窒息新生儿急性肾损伤(AKI)早期诊断中的价值。方法收集产科出生时有窒息的足月新生儿39例。其中符合2007年美国AKIN制定的AKI诊断标准12例(AKI组),无AKI 27例(非AKI组)。分别收集每例患儿出生后12、24、48、72 h血、尿标本,均检测血肌酐(Cr)、尿Cr、尿KIM-1。运用两样本t检验和直线相关分析对尿KIM-1诊断窒息新生儿早期肾损伤的价值进行分析。结果尿KIM-1与血Cr呈正相关(r=0.844,P<0.01);AKI组12 h与24 h血Cr差异无统计学意义(t=0.982 6,P>0.05);48 h血Cr高于24 h,差异有统计学意义(t=7.293 4,P<0.01);72 h血Cr明显高于48 h,差异有统计学意义(t=2.346 8,P<0.05)。非AKI组12、24、48 h血Cr差异无统计学意义(t=1.722 4,1.436 4,P>0.05);72h血Cr高于48 h,差异有统计学意义(t=4.879 4,P<0.05)。AKI组12、24、48、72 h尿KIM-1/尿Cr随时间逐渐升高,差异有统计学意义(t=7.997 4,4.948 0,2.896 5,P<0.05)。非AKI组24 h尿KIM-1/尿Cr高于12 h,差异有统计学意义(t=6.572 2,P<0.05);48 h及72 h尿KIM-1/尿Cr高于24 h,差异有统计学意义(t=6.572 2,4.857 2,P<0.05);48 h与72 h尿KIM-1/尿Cr比较差异无统计学意义(t=1.839 0,P>0.05)。AKI组与非AKI组比较,12及24 h血Cr差异均无统计学意义(t=0.360 2,-0.069 8,P>0.05);AKI组48及72 h血Cr明显高于非AKI组,差异有统计学意义(t=7.257 2,-4.478 7,P<0.01);AKI组与非AKI组比较,12 h尿KIM-1/尿Cr差异无统计学意义(t=0.454 4,P>0.05),AKI组24、48及72 h尿KIM-1/尿Cr均明显高于非AKI组,差异有统计学意义(t=4.268 0,5.992 1,6.427 5,P<0.01)。结论窒息新生儿AKI的发生后,尿KIM-1改变较血Cr更早,具有较好的早期诊断价值。
