连续性血液净化治疗急性肺水肿合并急性左心衰的疗效观察

Observation of clinical curative effect of continuous blood purification in the treatment of patients with acute pulmonary edema complicated with acute left heart failure

目的观察连续性血液净化治疗急性肺水肿合并急性左心衰的临床疗效。方法选择在我院治疗的急性肺水肿合并急性左心衰患者138例为研究对象,随机分为观察组及对照组两组,观察组(连续性血液净化组)70例,对照组(传统治疗组)68例,观察并比较疗效。结果观察组平均住院时间为(10.2±2.5)天、平均ICU住院时间为(3.8±1.0)天,均显著少于对照组;患者死亡率为1.4%、需行呼吸支持的患者比例为11.4%,均明显高于对照组。平均住院费用为(32120.5±1024.2)元,与对照组比较差异不明显(P>0.05)。结论连续性血液净化治疗急性肺水肿合并急性左心衰能够显著提高治疗的效果,但增加住院费用。

Objective To observe the clinical curative effect of continuous blood purification in the treatment of patients with a- cute pulmonary edema complicated with acute left heart failure. Methods From November, 2008 to February 2012, 138 patients with a- cute pulmonary edema complicated with acute left heart failure were selected and randomly divided into the observation group （ n = 70） and the control group （n = 68 ）. The observation group was given continuous blood purification and the control group was given the traditional treatment. The curative effect was observed and compared between the two groups. Results In the observation group, the average duration of hospital stay was 10. 2 ±2.5 days, and the average length of ICU stay was 3.8 ±1.0 days, which were both shorter than those in the control group with significant difference. The mortality was 1.4% and the patients who needed respiratory support was 11.4% in the ob- servation group, which were significantly higher than those in the control group （ P 〈 0. 05 ）. The average cost of hospitalization was 32120. 5± 1024. 2 Yuan in the observation group with no significant difference to that in the control group （ P 〉 0. 05 ）. Conclusion The continuous blood purification can improve the curative effect significantly as well as increase the cost of hospitalization in the treatment of patients with acute pulmonary edema complicated with acute left heart failure.
