儿童胸内淋巴结结核CT诊断分析

Diagnostic analysis of CT in the treatment of children with tuberculosis of intrathoracic lymph nodes

目的分析胸内淋巴结结核CT表现。方法对我院诊治120例胸内淋巴结结核进行分析。结果胸CT表现:肺门区淋巴结肿大共76例,右侧52例,左侧6例,双侧18例,炎症型37例,结节型39例,可发生在肺门上、中、下部。右肺门上部淋巴结肿多在上叶支气管分叉附近,中部淋巴结多在右肺动脉分叉附近,下部淋巴结在右肺下动脉干周围,伴有肺内播散8例。气管旁淋巴结肿大共44例,右侧40例,左侧3例,双侧1例,炎症型16例,结节型28例。结论儿童胸内淋巴结结核CT表现复杂,需认真分析,密切结合临床及实验室检查,可做出正确诊断。

Objective To analyze the CT charts of children with tuberculosis of intrathoracic lymph nodes. Methods The clini- cal data of 120 children with tuberculosis of intrathoraeic lymph nodes were retrospectively analyzed. Results The chest CT showed that 76 cases had lymphadenectasis around hilus of lung, which included 52 cases on the right, 6 cases on the left and 18 cases on the bilateral side, 37 cases had inflammation, and 39 cases had nodular, which occurred at the upper, central and lower hilus of lung. Lymph node enlargement of upper right hilus of lung mostly occurred near the upper lobe bronchus bifurcate. Lymph nodes of middle right hilus of lung mostly occurred near artery bifurcate. Lymph nodes of lower right hilus of lung mostly occurred around lower right pulmonary artery and there were 8 patients with spread in the lungs. There were 44 patients who showed trachea lymph nodes enlargement, which included 40 cases on the right, 3 cases on the right and 1 case on the bilateral sides. There were 16 cases of inflammation type and 28 cases of nodular type. Conclusion The CT images of children with tuberculosis of intrathoracic lymph nodes are complicated, which should be analyzed seriously and combined with clinical and laboratory examination to give a precise diagnosis.
