脑电双频指数监测下一次性可视喉镜在临床中的应用 被引量:4

摘要 目的探讨脑电双频指数监测下一次性可视喉镜在临床插管中对血流动力学的影响及插管并发症。方法择期全麻下行腔镜甲状腺手术女性患者60例,随机分为A组和B组,每组30例,全身麻醉诱导后A组使用Airtraq喉镜经口插管,B组采用MacIntosh弯喉镜经口插管;全身麻醉诱导和维持所用药物相同,监测比较两组声门暴露情况,建立气道时间,成功率以及插管并发症,记录患者入室后10 min(T0),气管插管前1 min(T1),插管后1 min(T2)、3 min(T3),拔管前1 min(T4)、拔管后1 min(T5)患者心率(HR)、平均动脉压(MAP)、心率-收缩压乘积(RPP)。结果与麻醉诱导前相比,两组诱导后即气管插管前的各项血流动力学指标较入室基础值均明显下降(P<0.05);气管插管后1 min较插管前1 min有明显升高(P<0.05),但两组间差异无统计学意义(P>0.05);拔管后1 min较拔管前血流动力学值有明显升高(P<0.05),但两组间差异无统计学意义(P>0.05);首次置入成功率两组比较差异无统计学意义(P>0.05);声门暴露Cormack and Lehane分级两组间差异有统计学意义(P<0.05);Airtraq光学可视喉镜较MacIntosh弯喉镜容易引起口腔黏膜损伤(P<0.05)及建立气道时间较长(P<0.05)。结论应用一次性可视喉镜能显著降低气道Cormack and Lehane分级,但建立气道时间较传统喉镜长;其不能避免对插管时血流动力学有影响,其自身性质易对口腔黏膜造成损伤。但考虑其结构特点,是一种安全有效的插管方式,其操作简单,在临床应用中值得推广。
