中西药序贯疗法对Hp阳性红斑痤疮疗效观察

Clinical Observation of Integrated Chinese and Western Medicine Sequential Therapy for Treatment of Hp-positive Acne Rosacea

目的:观察中西药序贯疗法对幽门螺杆菌(Hp)阳性脾胃湿热型红斑痤疮患者的临床疗效。方法:严格根据脾胃湿热型红斑痤疮的纳入标准选取Hp阳性的患者60例为研究对象,随机分为治疗组和对照组各30例,2组均进行三联疗法抗Hp治疗2周,疗程结束后治疗组再给予加减茵陈蒿汤治疗2周,4周后分别观察皮损评分改善情况,Hp指标及血清白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平的变化。结果:治疗组临床疗效与对照组比较,差异有显著性意义(P<0.05);治疗后治疗组Hp阳性率以及血清IL-8、TNF-α水平均降低明显,与对照组比较,差异均有非常显著性意义(P<0.01)。结论:红斑痤疮与Hp的感染关系密切;中西药序贯疗法能有效改善Hp阳性脾胃湿热型红斑痤疮患者的临床症状,降低Hp阳性率,降低血清TNF-α、IL-8水平。

Objective： To observe the therapeutic effect and mechanism of integrated Chinese and western medicine sequential therapy in the treatment of Hp-positive acne rosacea （AR） with spleen-stomach dampness-heat. Methods： Sixty qualified patients with sleepn-stomach damp- ness-heat were equally randomized into treatment group and control group. The two groups were given to triple anti-Hp therapy orally two weeks. After that, the treatment group was continually giv- en modified Yinchenhao decoction for 4 weeks. At the end of treatment, the improvement of AR skin lesion, and the changes of Hp-positive rate, serum interleukin 8（IL-8） and tumor necrosis factor al- pha（TNF-α） were investigated. Results： The clinical effective rate in treatment group was significantly better than that in control group（P 〈 0.05）. The Hp-positive rate, serum IL-8 and TNF-α were low- er in the treatment group than before treatment, and the decrease was superior to that in the con- trol group（P 〈 0.01）. Conclusion： AR has certain relation with Hp-positive rate. The sequential therapy of integrated Chinese and western medicine could effectively relieve the clinical symptoms of Hp-positive AR patients with spleen-stomach dampness-heat, and its mechanism is probably related to the decrease of Hp-positive rate and serum levels of IL-8 and TNF-α.
