奥曲肽联合乌司他丁治疗急性胰腺炎的疗效观察

Therapeutic efficacy of octreotide combined with ulinastatin in treatment of acute pancreatitis

目的分析奥曲肽联合乌司他丁治疗急性胰腺炎的疗效。方法将我院2009年4月至2011年4月所收治的63例急性胰腺炎患者作为研究对象,利用随机抽签法将所有患者分为观察组与对照组,其中对照组有30例患者,对他们给予0.1毫克的奥曲肽皮下注射,8h/次;观察组有33例患者,在对照组的治疗基础上对其给予10万U的乌司他丁与500毫升生理盐水或5%的葡萄糖注射液的混合液体静脉注射,8h/次,三天后给予5万U的乌司他丁静脉滴注。两组患者的1个疗程均为5～7天,对两组患者的平均住院时间、血淀粉酶恢复所需的时间、腹胀腹痛的缓解时间以及临床疗效进行比较。结果观察组的总有效率为93.9%,明显高于对照组的86.7%,他们之间的差异具有统计学意义,P<0.05;此外,观察组的平均住院时间、血淀粉酶恢复所需的时间、腹胀腹痛的缓解时间均比对照组短,他们之间的差异具有统计学意义,P<0.05。结论奥曲肽联合乌司他丁在治疗急性胰腺炎方面具有较好的疗效。

Objective To analyze the therapeutic efficacy of octreotide combined with ulinastatin in the treatment of acute pancreatitis. Methods Sixty-three patients with acute pancreatitis, who were admitted to our hospital from April 2009 to April 2011, were randomly divided into observation group （n = 33） and control group （n = 30）. The control group was subcutaneously injected with octreotide （0.1 mg） for 8 h each time; the observation group was intravenously injected with mixed liquid composed of ulinastatin （100000 U） and saline （500 ml） or 5% glucose injection for 8 h each time in addition to the therapy for control group and received ulinastatin by intravenous drip 3 d later. Each course of treatment for both groups was 5-7 d. The treatment outcomes of the two groups were compared in terms of mean hospital stay, time to recovery of blood amylase, and time to remission of abdominal distension and abdominal pain. Results The observation group had a significantly higher response rate than the control group （93.9% vs 86.7%, P 〈0.05）. Compared with the control group, the observation group had significantly shorter mean hospital stay, time to recovery of blood amylase, and time to remission of abdominal distension and abdominal pain. Conclusion Octreotide combined with ulinastatin have good therapeutic efficacy in the treatment of acute pancreatitis.
