不同剂量去氧肾上腺素静脉注射对腰麻下剖宫产产妇及新生儿的影响

摘要 目的探讨腰麻下剖宫产术中采用不同剂量的去氧肾上腺素静脉注射对产妇及新生儿的影响。方法本次研究选择的对象共60例,按P1组、P2组及C组各20例划分,P1、P2组给予不同剂量的去氧肾上腺素应用,与C组行等量生理盐水应用的产妇进行对照,对三组临床资料行回顾性分析。结果与入室后的相关指标加以对比,C组DBP、SBP在行腰麻操作后1、5min呈明显降低表现,差异有统计学意义(P<0.05)。P1组及P2组变化不明显。P1组、P2组在腰麻实施后1、5min和开展分娩前1minHR水平呈显著减慢表现,而C组仅开展分娩操作前1min呈减慢表现(P<0.05)。P1组出现高血压产妇1例,P2组出现高血压产妇3例。治疗后1min新生儿Apgar评分三组都有一定程度的提高,但P1、P2组5minApgar评分显著高于C组,差异有统计学意义(P<0.05)。结论腰麻下剖宫产术中采用小剂量去氧肾上腺素静脉注射具有较好的可控性,起效快,对产妇低血压可有效纠正,同时对胎儿及产妇造成的影响较小,明显降低了母婴并发症的发生率。
