经皮肾镜气压弹道超声碎石治疗32例肾结石

Percutaneous Nephrolithotomy with Pneumatic and Ultrasonic Lithotripter in treating Nephrolithiasis

摘要 目的探讨经皮肾镜气压弹道联合超声碎石、清石系统治疗肾结石的方法和临床效果。方法32例34侧肾结石患者均在B超定位引导下穿刺,扩张建立经皮肾通道,采用瑞士EMS三代气压弹道超声碎石机治疗。结果本组患者32例均一期穿刺取石成功,2例双侧结石采用分期穿刺取石。单侧手术时间40~90min。术后出现发热5例、迟发性出血1例、残余结石4例、双J管回缩2例,经妥善处理后治愈。结论经皮肾镜气雎弹道超声碎石、清石治疗肾结石损伤小、恢复快,安全、有效。 Objective To evaluate the process and effect of percutaneous nephrolithotomy with ultrasonic and pneumatic lithotripter nephrolithiasis. Methods With the guidance of ultrasonography, 34 renal units of 32 patients with nephrolithiasis were treated with EMS III Lithoclast. Results The percutaneous renal access was successful in all 32 patients and cured by percutaneous nephrolithotomy with ultrasonic and pneumatic lithotripter in first stage, 2 cases with double renal calculi were cured in second stage. Over all, the operation time of unilateral renal unit was 40-90 minutes; 5 cases complicated with postoperative fever; 1 case with delayed bleeding; residual stone was discovered in 4 cases; and the double J tube was retracted in 2 cases, they were all cured by correct treatment. Conclusion Percutaneous nephrolithotomy with ultrasonic and pneumatic lithotripter has the advantages of fewer complications and quick recovery. It was safe and effective.
