Report and Literature Review of Carcinosarcoma of Esophagus （6 cases）

目的探讨食管癌肉瘤的临床病理特征、组织起源及生物学行为。方法对6例食管癌肉瘤的临床病例资料进行回顾性研究，并通过伊红－苏木素（HE）、免疫组织化学染色检测，结合文献分析、随访。结果发现食管癌肉瘤X线检查表现为食管腔内巨大充盈缺损。食管镜检：多为鳞癌，病理特征为癌与肉瘤两种成分并存。免疫组化示：癌成分角蛋白（＋＋），部分癌细胞波形蛋白（＋＋）；肉瘤成分：波形蛋白（＋＋）。结论食管癌肉瘤可能起源干食管上皮的未分化储备细胞，其具有向上皮和间胚叶分化的潜力，并兼有两种组织源性细胞的特点。

Objective To explore the clinicopathological features, tissue origin, and biological behaviors of esophageal carcinosarcoma. Methods The clinical and pathological data of 6 cases with esophageal carcinosarcoma were analyzed, retrospectively by HE and immunohistochemistry.We studied the cases after-treatment. Results The lesion displayed a large filling defect in X-ray imaging. The pathological diagnosis was squamous carcinoma; the pathological feature showed the disease composed of carcinomatous and sarcomatous elements. Immunochemically, the carcinomatous elements were positive to Keratin in all the cases and a part of carcinomatous elements were positive to Vimentin. In all cases, the sarcomatous elements were positive to Vimentin. Conclusion The study suggested that cacinosarcoma of the esophagus originate in the puripotent stei cell, has the potential to differentiate into separate carcinomatous and sarcomatous participants.
