特重度烧伤休克期患者伴行股动脉、股静脉血气指标监测及意义 被引量:7

摘要 目的探讨伴行股动、静脉血气分析对烧伤休克复苏治疗的指导意义。方法 23例特重度烧伤(烧伤总面积≥90%TBSA,Ⅲ度烧伤面积≥50%TBSA)合并吸入性损伤患者,其中治愈14例(治愈组),死亡9例(死亡组),比较两组休克期液体入量及尿量,入院即刻及伤后24、48 h伴行股动脉、静脉的血气指标。结果死亡组伤后第1个24 h液体入量多于治愈组,P=0.025,伤后24 h股动脉PaCO2高于治愈组,P=0.002;三个时相的股静脉氧分压(PvO2)低于治愈组,P<0.05,三个时相的股静动脉二氧化碳分压差值(VAPCO2)高于治愈组,P<0.05;两组休克期尿量比较无统计学意义。结论特重度烧伤休克复苏中过量补液可导致伴行动静脉血气指标改变,从而影响患者预后。
