哮喘患者支气管肺泡灌洗液螨特异性IgE、IgG_4测定临床意义研究 被引量:5

The Effect of Mite-specific Immunoglobin G_4 in Human Asthma

摘要 为探讨IgG4 在哮喘发病中的应用 ,寻求哮喘的免疫学分型的实验依据 ,采用皮肤过敏原点刺试验筛选螨过敏性哮喘 30例 ,分别检测患者急性发作期、临床好转期、特异性脱敏治疗后三个阶段 ,支气管肺泡灌洗液 (BALF)和血液中disIgE、disIgG4 的含量 ,观察BALF和血液中disIgE、disIgG4 水平之间的相互关系 ,及其在上述三阶段中disIgE、disIgG4 的动态变化规律。显示 30例实验组中急性期BALF和血液中单纯disIgG4 增加 4例 ,单纯disIgE增加 12例 ,disIgE和disIgG4 均增加 14例。特异性脱敏治疗后BALF及血液中disIgE明显下降 (P <0 0 1) ,而disIgG4 增加 (P <0 0 1)。对照组BALF及血液中未检测到disIgE、disIgG4 ,说明⑴IgG4 是介导Ⅰ型变态反应的过敏性抗体之一 ;⑵IgG4 To investigate the role of mite-specific immunoglobulin G 4 in patients with asthma,we selected 30 cases of allergic asthma induced by mites through allergen pick test.The mite-specific dis IgE,dis IgG 4 contents were determined both in the bronchoalveolar lavage fluid(BALF) and blood at acute attack,clinical relief phases and after specific desensitization treatment.The relationship between dis IgE and dis IgG 4 in BALF and blood was observed at the three phases.The results showed that of 30 cases,4 cases had an elevation of dis IgG 4 in acute phase in BALF and blood,12 cases,dis IgE increased,14 cases had both dis IgE and dis IgG 4 increased.After specific desensitization treatment,the dis IgE distinctly decreased in BALF and blood(P<0 01),whereas the dis IgG 4 was increased(P<0 01).In the control group the dis IgE and dis IgG 4 were undeterminable.Therefore,we consider that the IgG 4 is one of the antibodies mediated type-Ⅰ allergic reaction,the IgG 4 could guard body by preventing the synthesis of IgE.
