干燥综合征患者腮腺扩散加权成像研究 被引量:8

摘要 目的探讨扩散加权成像在干燥综合征患者腮腺病变中的诊断价值。资料与方法对8例女性腮腺干燥综合征患者及8名健康志愿者行腮腺扩散加权成像扫描,并计算表观扩散系数(ADC)值。分析两组受试者腮腺ADC值的差异以及腮腺ADC值与患者病程的相关性,并采用受试者工作特征曲线进行诊断点分析。结果 8例患者腮腺体积正常,常规影像及扩散加权成像扫描均未见异常信号。干燥综合征患者组腮腺平均ADC值为(0.89±0.14)×10-3mm2/s,显著低于正常对照组的(1.13±0.22)×10-3mm2/s(P<0.05)。干燥综合征患者腮腺ADC值与病程呈显著负相关(r=-0.79,P<0.05)。受试者工作特征曲线分析提示曲线下面积为0.83,最佳诊断点为0.99×10-3mm2/s。结论扩散加权成像可以用来评估干燥综合征患者的腮腺损伤。 Purpose To investigate the diagnostic value of diffusion weighted imaging (DW1) in the parotid glands of patients with Sjogren's syndrome. Materials and Methods Diffusion weighted imaging was performed in 8 patients with Sjogren's syndrome and 8 health controls, and apparent diffusion coefficient mapping was calculated. The difference of ADC value between the two groups and correlation between ADC value and course of the disease were analyzed, then receiver operating characteristic (ROC) curve was used to get the diagnostic ADC value. Results The volume ofparotid glands of 8 patients were normal, and MR signal was normal on conventional MR imaging and DWI. The mean ADC value oflhe parotid glands of the patients with Sjogren's syndrome (0.89_+ 0.14) X 10~ mm2/s was significantly lower than that of health controls ( 1.13 ! 0.22)X 10-~ mm2/s (P〈0.05). The negative correlation (r -- 0.79, P〈0.05) was detected between ADC value and disease duration for the patients. ROC curve analysis demonstrated that the area under the curve was 0.83, and the best diagnostic cut-off point of ADC value was 0.99 10-3 mm2/s lbr the patients with Sjogren's syndrome. Conclusion DWI can be used to evaluate the impairment of the parotid glands in patients with Sjogren's syndrome.
