小脑幕切开对缓解侧裂区胶质瘤术后脑水肿及降低颅内压的相关性研究 被引量:9

摘要 目的探讨对于以侵犯侧裂区为主的胶质瘤,术中将小脑幕切开,是否能有效预防术后出现术区水肿,避免高颅压脑疝形成。方法本研究共收集侧裂区胶质瘤患者62例,随机分为实验组和对照组。其中,实验组患者32例,为缓解术后水肿,防止术后高颅压形成脑疝,术中在切除病灶后将小脑幕裂孔切开约1cm,并充分止血后关颅。对照组患者30例,术中未将小脑幕切开。统计患者术后颅内压指数,术后住院时间,甘露醇用量,二次手术(术后水肿导致高颅压形成脑疝行去骨瓣减压术)例数,及术后急性肾功能损害情况,并进行统计学分析。结果实验组术后颅内压(11.03±2.82)mmHg,术后住院时间(10.96±1.49)的和甘露醇用量(28.84±4.08)次均小于对照组[分别是(16.35±3.69)mmHg,(12.40±1.65)d,(37.26±4.63)次],差异有统计学意义(P<0.05)。结论对于侧裂区胶质瘤,为预防术后脑水肿所致的脑疝,将小脑幕切开,可起到重要的作用,减少了术后并发症对患者的影响。
