植入体内多孔涂层假体界面研究

Study on interface of retrieved porous coated prostheses

目的 研究翻修取出的假体柄多孔涂层内骨长入情况 ,评定X线平片判断假体稳定的应用价值。方法 假体编号后经丙酮脱水固定 ,低温塑料包埋 ,切割出 3mm厚片及 10 0 μm以下薄片。厚片用软X射线高清晰度胶片显微拍照 ,薄片甲苯胺蓝染色后光镜下观察。结果 显微摄片显示 :临床X线片认为骨长入稳定型假体有的部分涂层内无骨长入 ,有的则是纤维稳定型。光镜发现骨未长入部位有纤维组织间隔 ,骨长入比例大小在各个假体不尽相同。有的假体涂层表面见到脱落的涂层颗粒及吞噬磨屑的巨噬细胞。结论 假体柄多孔涂层设计与骨长入数量紧密相关 ;X线平片判断假体稳定的价值是有限的。

Objective To study the bone ingrowth into the porous coating of retrieved femoral prosthesis and evaluate clinical value of X ray film in judging the stability of the prostheses. Methods\ After they were labeled at first, all prostheses were dehydrated by acetone and buried by MMA, and sectioned into 3 mm and about 100 μm slices. The former was roentgenographied with high resolution film,the latter was stained with toluidine and observed with light microscope. Results\ On the high resolution film, the PCA and Ti 6Ai 4V porous coating shown complete bone ingrowth by X ray film had no bone ingrowth, national“pearl coating” was fibrous ingrowth. Bone ingrowth percentage was different in different prosthesis. Wear particles and macrophage were found in some interfaces. Conclusion The porous coating design was related to bone ingrowth quantity the value of X ray film is limited in diagnosing the bone ingrowth into the porous coating
