不同血糖水平对2型糖尿病患者肺功能改变的影响

Influence of Blood Glucose on the Pulmonary Function of Patients with Type 2 Diabetes

目的检测不同血糖水平及糖化血红蛋白水平对2型糖尿病(T2DM)患者肺功能的影响。方法 130例T2DM患者分别根据空腹血糖水平及糖化血红蛋白水平高低各分为3组,分别检测各组肺通气、弥散功能。对照组为本院体检健康人群40例。结果与对照组比较,轻度和中度T2DM组的肺通气功能(VC、FEV1、MVV、TLC)及弥散功能均无明显改变(P>0.05)。重度T2DM组在肺通气功能(VC、FEV1、MVV、TLC)及弥散功能(DLco)方面均较对照组有显著性降低(P<0.01)。低HbAlc组的肺通气功能(VC、FEV1、MVV、TLC)及弥散功能无明显改变(P>0.05);中、高HbAlc组在肺通气功能(VC、FEV1、MVV、TLC)及弥散功能(DLco)均较对照组有显著性降低。结论 T2DM患者中,过高血糖水平及糖化血红蛋白高于正常均能加重肺功能损害,及时合理控制血糖可能是降低肺功能损害的重要措施。

Objective To evaluate the influence of blood glucose and glycosylated hemoglobin on pulmonary functions of type 2 diabetes patients. Methods According to blood glucose or glycosylated hemoglobin （HbAlc） levels,130 patients with type 2 diabetes mellitus（T2DM） were divided into mild, moderate, and severe groups, respectively. The pulmonary ventilation and diffusion capacity of these 130 patients and 40 healthy objects were measured. Results Compared with control, the pulmonary ventilation （ VC, FEV1, MVV, TLC） and diffusion capacity（DLco） of mild and moderate T2DM patients had no significant change（P〉0. 05）. However, the pulmonary ventilation（VC, FEV1, MVV,TLC） and diffusion capacity（DLco） of severe T2DM had decreased significantly（P〈0.01 ）. Moreover, the pulmonary ventilation （ VC, FEV1, MVV, TLC） and diffusion capacity （DLco） had decreased significantly on moderate and high HbAle patients but not on low HbAlc patients compared with control. Conclusion In type 2 diabetic patients,high blood glucose and above normal HbAlc are harmful for pulmonary function. Thus, timely and appropriately control of blood glucose would he an important strategy to reduce the damage of pulmonary function.
