溃疡性结肠炎缓解期患者5-氨基水杨酸维持治疗复发的相关因素分析

Related Factors to Recurrence of Ulcerative Colitis in Patients with 5-Aminosalicylic Acid Therapy for Maintenance of Remission

目的分析溃疡性结肠炎缓解期患者5-氨基水杨酸维持治疗复发的相关因素。方法收集2003年2月-2012年2月112例应用5-氨基水杨酸进行维持治疗的溃疡性结肠炎缓解期患者的临床资料,对可能导致溃疡性结肠炎复发的因素,包括性别、病程、病情、病变分型、是否应用激素药物、口服5-ASA的剂量及黏膜愈合情况,先行χ2检验进行单因素分析,对有意义的指标再行COX多元回归分析。结果本组复发50例,复发率为44.64%。溃疡性结肠炎缓解期患者5-氨基水杨酸维持治疗后复发与病程、病情程度、是否用过激素药物、口服药物的剂量及黏膜愈合情况有关(P均<0.05);COX回归多因素分析结果显示,黏膜愈合情况及病情程度与溃疡性结肠炎的复发高度相关(P均<0.05)。结论黏膜愈合情况及病情程度是溃疡性结肠炎缓解期患者5-氨基水杨酸维持治疗复发的重要因素。

Objective To analyze the related factors of the recurrence of ulcerative colitis in patients with 5-aminosalicylic acid therapy for maintenance of remission.Methods The clinical data of 112 patients with ulcerative colitis receiving 5-aminosalicylic acid therapy for maintenance of remission were collected from February 2003 to February 2012 in our hospital.The chi-square test was utilized for the analysis of the related factors including gender,disease duration,disease,lesion type,whether used hormone drugs,the oral dose of 5-ASA and mucosal healing primarily,then the significant indexes were analyzed by the multiple regression analysis.Results The recurrence rate was 44.64%.The related factors including the course of the disease,severity of disease,whether used hormonal drugs,oral medication dose and mucosal healing showed correlation with the recurrence of the patients with remission of ulcerative colitis treated with the maintenance therapy of 5-amino salicylic acid（P0.05）;Mucosal healing and severity of disease showed high correlation with the recurrence（P0.05）.Conclusion Mucosal healing and severity of disease are the most two important factors of the recurrence of the patients with remission of ulcerative colitis receiving 5-aminosalicylic acid therapy for maintenance of remission.
