以中枢神经系统症状为首发表现的肺癌脑转移瘤MR特征分析

MRI Features of Brain Metastases of Lung Cancer with Central Nervous System Problem as Firstly Symptoms

目的研究分析以中枢神经症状为首发的肺癌脑转移瘤的磁共振特征及诊断价值。方法收集研究组89例以中枢神经症状为首发的肺癌脑转移患者和对照组97例以呼吸症状为首发的肺癌脑转移患者行常规MR及增强检查,分别对转移瘤好发部位、瘤周水肿程度、转移瘤数目、强化方式程度及增强后病灶的显示效果进行对比、观察、分析。结果研究组相对于对照组在转移瘤好发部位,瘤周水肿程度及强化方式上存在差异,两组在增强后均更易发现平扫时未确切显示的新病灶,在转移瘤数目上两者不存在差异性。结论以中枢神经症状为首发的肺癌脑转移瘤与以呼吸系统症状为首发的肺癌脑转移瘤在MR诸多表现上存在差异,通过对此类转移瘤研究、观察分析,其特征表现可与其他病变相鉴别诊断。

Objective To observe the MRI features of brain metastases of lung cancer with central nervous system problem as the firstly symptoms and discuss its diagnosis values.Methods All 89 patients with brain metastases of lung cancer with central nervous system problem as the firstly symptoms were as study group,and 97 patients with brain metastases of lung cancer with respiration system problem as the firstly symptoms were as study group.The conventional and enhance MRI were performed,and the sites of predilection,peritumoral edema,numbers of metastatic tumor,enhanced MRI mode and its manifestations were recorded and analyzed.Results There were differences in predilection sites,peritumoral edema and the features of enhancement between the two groups.The new focuses can be easily found by the enhanced MRI as compared to the plain MRI.There was no difference in the numbers of metastatic tumor.Conclusion There is obviously difference in MRI features between brain metastases of lung cancer with central nervous system problem as the firstly symptoms and brain metastases of lung cancer with respiration system problem as the firstly symptoms.The conventional and enhanced MRI can significantly improve the detection rate of metastases,and distinguish them from other lesions.
