腹腔镜联合胆道镜胆总管探查取石一期缝合术32例治疗体会

Clinical value of laparoscopic common bile duct exploration with choledochoscope and primary suture of common bile duct:with a report of 32 cases

目的探讨腹腔镜联合胆道镜胆总管探查取石一期缝合术治疗胆石症的临床价值。方法回顾分析2011年3月至2012年3月我院32例施行腹腔镜联合胆道镜胆总管探查取石一期缝合术的临床资料。结果 32例手术均成功,手术时间60～180 min,其中术后胆瘘1例,经腹腔引流5 d后停止,住院时间5～14 d。结论腹腔镜联合胆道镜胆总管探查取石一期缝合术安全、有效,患者术后恢复快,可作为治疗胆总管结石的常规术式。

Objective To evaluate the clinical value of laparoscopic common bile duct exploration with choledochoscope and primary suture of common bile duct. Methods The clinical data of 32 cases undergoing this operation between March 2011 and March 2012 was summarized and retrospectively analyzed in our hospital. Results All operations were successfully performed. The operation time was 60 - 180 min. The bile leakage occurred in one case which ended after 5 d by peritoneal drainage,and the postoperative hospital stay was 5 - 14 d. Conclusions Laparoscopic common bile duct exploration with choledochoscope and primary suture of common bile duct is safe and effective and can help patients rapid recovery, which could be a routine method for common bile duct stones.
