残角子宫的诊断及处理(附21例临床分析)

Rudimentary horn of uterus:diagnosis and treatment

目的 总结残角子宫的临床特征 ,探讨诊断和处理方法。方法 分析经手术确诊的残角子宫病例 2 1例 ,其中残角子宫妊娠组 13例 ,非妊娠组 8例。结果 临床特征有痛经、盆腔包块、急腹症、早孕人工流产吸空。 14例单纯切除残角子宫 ,术后无并发症发生 ,6例仅行残角子宫侧输卵管结扎或切除 ,其中 2例术后继发子宫内膜异位症 ( 1例还伴发残角子宫积血 )。结论 早孕人流吸空和孕中期出现急腹症应警惕残角子宫妊娠 ,无论合并妊娠与否 ,一旦确诊 。

Objective Studying the diagnosis and treatment of rudimentary horn of uterus by analysising the clinical characteristics of 21 cases.Methods All thd 21 cases were diagnosed by operation,of which 13 patients were complicated with pregnancy.Results All the cases demonstrated these characters such as menorrhalgia,pelvic tumors,acute abdomen and failure of abortion in early pregnancy.Fourteen patients with acute abdomen were carried out the amputation of the rudimentary parts without any complications.Among six women with ligation of oviduct or tubectomy,two were complicatied with endometriosis.One of the two patients showed cornual hematocele.Conclusions Failure of abortion in early pregnancy and acute abdomen in midterm pregnancy should be highly suspected with cornual pregnancy.Once accurately diagnosed,whether pregnant or not,the rudimentary part and its tube should be cut off at the same time.
