彩色多普勒超声对乳腺良恶性肿块的诊断价值

Value of color Doppler ultrasound in the diagnosis of benign and malignant breast lumps

目的探讨彩色多普勒超声对乳腺良恶性肿块的诊断价值。方法选取2010年3月～2011年12月来我院进行治疗的76例乳腺良恶性肿块患者,全部均经手术和病理证实,使用sEQuOIA-512型彩色多普勒超声仪,探头频率7～13 MHz。患者取仰卧位,观察肿块的部位、形态、大小、边界、内部回声以及应用彩色多普勒(CDFI)观察血流Adler分级并测量阻力指数(RI)。结果 50例乳腺良性肿块患者中,乳腺腺瘤样增生32例,纤维瘤18例。26例乳腺恶性肿块患者中,浸润性导管癌10例,单纯癌13例,髓样癌、导管内癌及浸润性腺癌各1例。乳腺良恶性肿块的血流Adler分级及RI比较存在显著性差异(P<0.05)。结论彩色多普勒超声对乳腺良恶性肿块的诊断具有重要的临床价值。

Objective To investigate the value of color Doppler ultrasound in the diagnosis of benign and malignant breast lumps. Methods A total of 76 cases benign and malignant breast masses were selected from March 2010 to December 2011 in our hospital for treatment, all with surgically and pathologically confirmed sEQuOIA-512 type color Doppler ultrasound system, transducer frequency 7 -13 MHz. Supine position in patients with observed tumor site, shape, size, boundary, internal echo color Doppler （CDFI） observed blood flow Adler classification and measurement of the resistance index （RI）. Results Fifty cases of benign breast tumor patients, including breast adenoma-like hyperplasia 32 cases, 18 cases of fibroma. 26 cases of breast malignant tumor patients, each of the 10 cases of invasive ductal carcinoma, 13 cases of simple cancer, medullary carcinoma, ductal carcinoma in situ and invasive adenocarcinoma. Benign and malignant breast tumor blood flow the Adler grading and the RI compare the presence of a significant differ ence （P 〈 0.05）. Conclusion Color Doppler ultrasound has important clinical value in the diagnosis of benign and malignant breast lumps
