阴性、阳性症状为主的精神分裂症患者事件相关电位的比较

Comparative Study of ERP(N100,P200,N200,P300) in Positive and Negative Subtypes of Schizophrenics

目的探讨阴性、阳性症状为主的精神分裂症患者是否有不同的脑功能异常。方法将 2 2例阴性、2 7例阳性初发未服药的精神分裂症患者和 17例正常对照组 ,进行事件相关电位 (N10 0 ,P2 0 0 ,N2 0 0 ,P30 0 )测查。结果阴性组在Fz ,Cz ,Pz ,T7,T8五个点的P30 0波幅显著低于和潜伏期明显长于正常对照组 ,阳性组在Cz ,Pz点的P30 0波幅低于正常对照组 (P <0 .0 5 ) ,P30 0的波幅与潜伏期两组患者之间差异有显著性 (P <0 .0 5 ) ;阴性患者N 10 0、N2 0 0的波幅低于正常对照组 ;相关分析发现阴性分量表分与Fz的P30 0波幅有显著负相关 (r =-.31,P <0 .0 5 )、与T7,T8。点的潜伏期呈显著正相关 (r =.33 ,r =.35 ,P <0 .0 5 )。结论阳性、阴性精神分裂症可能有不同的神经生物学基础。

Objective To study whether there were different functional abnormalities of brain in the negative and positive subtypes of schizophrenia. Methods N100,P200, N200 and P300 of event-related potentials (ERP) were measured in twenty-two negative subtypes and twenty-seven positive subtypes who were still in drug free episode and met the CCMD-II-R schizophrenic criteria. Seventeen normal subjects served as controls. Results The wave amplitudes of P300 at five points (Fz,Cz,Pz,T 7 and T 8) in the negative subtypes were significantly lower than those of normal controls; whereas the latent periods of their P300 were distinctly longer than those of control group. The wave amplitudes of P300 at Cz and Pz points in the positive subtypes decreased as compared to normal controls (P<0.05). Significant negative correlation existed between negative syndrome and the P300 amplitude at Fz in schizophrenia (r=-0.31, P<0.05). Significant positive correlation between it and the latent periods at T 7,T 8 was also observed (r=0.33 and 0.35, P<0.05). Conclusion These findings suggest that the negative and positive subtype of schizophrenics possibly display different basis of neurobiology.
