慢性阻塞性肺疾病运动能力及运动性肺动脉高压测定的意义

摘要 目的 应用运动心肺负荷试验早期检出慢性阻塞性肺疾病 (COPD)运动性低氧血症及运动性肺动脉高压。方法 实验组分为 2组 ,慢性阻塞性肺疾病组 62例 ,正常对照组 3 0例 ,用HPSONS 10 0 0型彩色多普勒超声仪 ,采用三尖瓣返流法于运动前后分别测定 2组受试者的肺动脉收缩压。用Jaeger公司产运动心肺功能仪 ,采用连续递增负荷方案测定最大摄氧量 (VO2 max) ,最大功率 (WATT)等。结果 COPD患者运动能力较对照组明显减低 ,运动后动脉氧分压降低 ,肺动脉收缩压增高 (P <0 .0 5 )。结论 COPD患者运动能力下降 ,运动心肺功能试验可早期发现低氧血症及肺动脉高压。 Objective Cardiopulmonary exercise test is used for early detection of exercise induced hypoxia and exercise induced pulmonary arterial hypertension of chronic obstructive pulmonary disease (COPD).Methods The experiment included two groups, COPD group with 62 cases and normal control group with 30 cases. Pulmonary arterial systolic pressure (PASP) before and after the exercise test was detected on the subjects of the two groups by tricuspid regurgitation technique with HPSNOS 1000 color doppler echocardiogram instrument. Maximum oxygen intake (VO 2max), maximum work allowance (WATT) and others were measured with a cardiopulmonary function instrument made by Jaeger company through a continuous Load adding method, ramp pattern plan.Results Exercise performace of COPD was significantly lower than that of normal control group, COPD patients after the exercise showed lower partial pressure of oxygen in arterial blood, and higher PASP(P< 0.05 ).Conclusion Due to the COPD patients’lower exercise performance, the cardiopulmonary function test can be used in early detection of hypoxia and pulmonary arterial hypertension.
