缺血损伤脊髓运动诱发电位的变化及其意义 被引量:7

摘要 目的 探讨缺血再灌流损伤脊髓运动诱发电位 (MEP)的变化规律及其意义。方法  2 4只家兔随机均分为对照组、缺血 3 0、60、90min组 ,以选择性腰动脉阻断模拟脊髓缺血再灌流损伤 ,观察各组在缺血前、缺血时及再灌流后MEP的变化。结果 缺血 ( 2 0 .6± 5 .7)min时MEP消失 ,各组家兔再灌流时MEP的N1和P1波峰潜时总体变化趋势为缺血 90min组 >缺血 60min组 >缺血 3 0min组 >对照组。结论 脊髓缺血时间越长 ,再灌流损伤越重 ,脊髓运动传导功能损害越明显。MEP对损伤脊髓运动功能和预后判断具有重要价值。 Objective To investigate changes of motor evoked potentials(MEP)of spinal cord following ischemic and reperfusional injury and its significance.Methods 24 rabbits were randomly and equally divided into the control group、ischemia for 30,60 and 90 min group.The model of ischemic and reperfusional injury of spinal cord was simulated by the selective occlusion of the lumbar arteries, MEP during the preischemia,ischemia and reperfusion were recorded continually in the every group.Results The MEP was obliterated at the ( 20.6 ± 5.7 ) min of ischemia.The latencies of peak both N 1 and P 1 wave in MEP during reperfusion from the longest to the shortest in the order were generally ischemia for 90min group, ischemia for 60min group,ischemia for 30 min group,the control.Conclusion These results showed the longer the ischemic time, the more severe the reperfusional injury, and the more dysfunctional the motor function of spinal cord. It is very important for MEP to estimate the motor function and prognosis of injured spinal cord.
