摘要
目的:探讨用乳癌组织中E-cadherin和CD44V6的表达水平预测乳癌腋窝淋巴结转移的价值。方法:用免疫组化法检测98例乳癌(浸润性导管癌)患者手术切除标本中E-cadherin和CD44V6的表达情况,以术后病理检查结果为金标准,根据E-cadherin和CD44V6的表达水平预测腋窝淋巴结转移情况,分析单一因素和联合模式预测的灵敏度、特异度、误诊率和漏诊率等指标。结果:E-cadherin单一因素预测腋窝淋巴结转移的灵敏度为60.71%,漏诊率为39.29%,特异度为92.86%,误诊率为7.14%;CD44V6单一因素预测腋窝淋巴结转移的灵敏度为64.29%,漏诊率为35.71%,特异度为61.91%,误诊率为38.10%;E-cadherin和CD44V6串联模式预测的灵敏度为41.07%,漏诊率为58.93%,特异度为95.24%,误诊率为4.76%;E-cadherin和CD44V6并联模式预测的灵敏度为83.93%,漏诊率为16.07%,特异度为59.52%,误诊率为40.48%。结论:E-cadherin单一因素和E-cadherin与CD44V6串联模式预测乳癌淋巴结转移的特异度高,可为腋窝淋巴结清扫术的取舍和术后治疗方案的选择提供帮助;E-cadherin与CD44V6并联能提高淋巴结转移的检出率。
Aim:To analyze the value of the expressions of E-cadherin and CD44V6 in carcinoma tissue for evaluating axillary lymph node metastasis of breast cancer.Methods:The expressions of E-cadherin and CD44V6 in carcinoma tissue of 98 cases of primary invasive ductal carcinoma were detected by immunohistochemistry.The sensitivity,specificity,misdiagnosis and false negative rate compared with the pathology results were calculated.Results:Based on the results of pathology,for the evaluation of axillary lymph node metastasis,the sensitivity of E-cadherin was 60.71%,the false negative rate was 39.29%,the specificity was 92.86% and the false positive rate was 7.14%;the sensitivity of CD44V6 was 64.29%,the false negative rate was 35.71%,the specificity was 61.91%,and the false positive rate was 38.10%.In the series model of E-cadherin and CD44V6,the sensitivity was 41.07%,the false negative rate was 58.93%,the specificity was 95.24%,and the false positive rate was 4.76%.In the parallel model of E-cadherin and CD44V6,the sensitivity was 83.93%,the false negative rate was 16.07%,the specificity was 59.52% and the false positive rate was 40.48%.Conclusion:The single factor of E-cadherin and the series model of E-cadherin and CD44V6 have high specificity,and could provide help for the choose of axillary lymph node dissection and the treatment programs after surgery.The parallel model of E-cadherin and CD44V6 could increase the detection rate of lymph node metastasis.
