彩色多普勒超声诊断胎儿唇腭裂的临床应用

Clinical diagnosis of color doppler ultrasound in the cleft-lip and cleft-palate

目的研究彩色多普勒超声诊断胎儿唇腭裂的临床应用。方法应用彩色多普勒超声(彩超)诊断仪对妊娠18～40周孕妇2 421例(共计胎儿2 464例)进行产前常规检查及畸形筛选,对诊断为唇腭裂的病例进行跟踪随访,对比足月生产或引产结果。结果 14例唇腭裂患儿中应用彩超检查出唇腭裂13例,其中单纯唇裂3例,单纯腭裂1例,唇裂伴发腭裂9例。漏诊1例单纯不完全性腭裂漏诊,1例完全性唇裂误诊为不完全性唇裂,彩超敏感率92.86%,诊断唇腭裂分类符合率为92.31%。结论彩色多普勒超声作为常规产前检查,对于筛选唇腭裂畸形具有较高的敏感性,准确率高,安全,简便,对提高人口素质具有重要意义。

[Objective ] To study clinical diagnosis of color doppler ultrasound in the cleft-lip and cleft- palate. [Methods] The regular prenatal examination and screening abnormal of 2 421 gravida （2 464 fetus） whose gestational period from 18th to 40th week were examined by color doppler ultrasound. The cases diagnosed cleft-lip and cleft-palate were to tracking survey for compared with autopsy or newborn. [Results] Among 14 cases of children patients with cleft lip and palate, 13 facial deformities were detected, and 3- cases were diagnosed as CL, 1-case was diagnosed as CP and 9 cases were diagnosed as CLCP. 1-case of simple incomplete cleft palate was missed and 1 case of complete cleft-lip was misdiagnosed as unilateral cleft lip. The sensitivity was 92.86% and coincide rate was 92.31%, compared with autopsy or newborn. [Conclusion] As a routine prenatal examination, color doppler ultrasound were high sensitivity, high accuracy, safe, simple to screening cleft-lip and cleft-palate, significant to improving the quality of population.
