MSCT对阑尾黏液囊肿的诊断与鉴别诊断

Multislice CT diagnosis of appendiceal mucocele and mucinous tumors

目的探讨阑尾黏液囊肿的MSCT特征及其诊断价值。方法回顾性分析3例阑尾黏液囊肿的MSCT征象,并选取6例阑尾黏液性肿瘤进行对比。结果 3例阑尾黏液囊肿CT表现为圆形、椭圆形或长条形囊性密度包块,囊壁菲薄,无强化。黏液性肿瘤6例(黏液囊腺瘤5例,黏液囊腺癌1例);5例黏液囊腺瘤CT表现为囊性密度包块,囊壁轻度均匀增厚,1例囊壁有钙化;增强后囊壁不强化或轻度均匀强化。1例黏液囊腺癌,表现为囊壁不规则增厚,有不均匀强化。结论阑尾黏液囊肿的MSCT表现有一定的特征性,在其良、恶性鉴别上有一定的价值。

Objective To evaluate the multislice CT（MSCT）characteristics of appendiceal mucoceles and mucinous tumors. Methods MSCT of appendieeal mucoceles （3 patients） was compared with that of mutinous tumors （6 patients）. Results Appendiceal mucoceles were round, oval or tubular cystic lesions with thin wails and no contrast enhancement. The mueinous tumors included mutinous adenomas （5）and mutinous adenocarcinoma （1）. The mutinous adenomas appeared as cystic masses with heterogeneously enhancing regular thick walls. Mural calcification was found in lof the 5 patients. In one patient with mucinous adenocarcinoma, the tumor wall was irregular and thick with heterogeneous contrast enhancement. Conclusions MSCT characteristics of appendiceal mucoceles and mucinous tumors are helpful in differentiating benign from malignant lesions.
