前臂游离皮瓣修复口腔癌术后组织缺损患者的生活质量评估 被引量:15

Assessment of quality of life of oral cancer patients after reconstruction with radial forearm free flaps

摘要 目的评估口腔颌面部恶性肿瘤切除术后应用前臂游离皮瓣修复组织缺损患者的生活质量。方法采用简明健康状况调查问卷(the medical outcomes study-short form-36,SF-36)和口腔健康影响程度量表(the14-item oral health impactprofile,OHIP-14)对口腔颌面部恶性肿瘤切除术后12个月以上的49例患者进行生活质量调查和评估。结果发出问卷49份,回收41份,共完成问卷调查84%。SF-36调查问卷得分高的项目依次是躯体角色[(92.9±2.6)分]和身体疼痛[(82.6±5.7)分];得分较低的项目是活力[(61.5±9.1)分]、情感角色[(64.9±6.8)分]和社会功能[(65.2±8.2)分]。OHIP-14量表得分好的项目依次是残障[(37.1±15.1)分]及心理障碍[(45.7±11.9)分],得分差的项目依次是生理性疼痛[(64.2±11.7)分]、功能限制[(61.9±12.9)分]和心理障碍[(59.2±12.1)分]。结论应用前臂游离皮瓣修复口腔颌面部恶性肿瘤切除术后组织缺损,对患者生活质量有显著的影响。 Objective To evaluate the quality of life in patients who had resection of oral cancer and reconstruction by radial forearm free flaps. Methods Quality of life of 49 patients was assessed by means of the 14-item oral health impact profile (OHIP-14) and the medical outcomes study-short form-36 (SF-36) questionnaires 12 months after operation. Results Forty-one questionnaires were collected (84%). SF-36 : the highest-scoring domain were physical role ( 92. 9 ± 2. 6 ) and bodily pain ( 82. 6 ± 5.7 ) , the lowest-scoring domain were vitality ( 61.5 - 9. 1 ), followed by role emotion ( 64. 9 ± 6. 8 ) and social functioning ( 65.2 ± 8.2 ) . OHIP-14 : the best-scoring domain were handicap ( 37.1 ± 15.1 ) and psychological disability ( 45.7 ± 11.9 ), the best-scoring domain were physical pain ( 64. 2 ± 11.7 ) and functional limitation (61.9 ± 12. 9). Conclusions Radial forearm free flaps for reconstruction of oral defects after cancer resection could significantly influence the patients' quality of life.
