摘要
目的研究Mcl-1、Bax在肝细胞癌及癌旁肝组织中表达的关系及临床意义,探讨Mcl-1表达与肝癌组织学分级及预后的关系。方法对48例人肝细胞癌及相应的癌旁肝组织标本,进行免疫组织化学EnVisionTM法检测。结合Mcl-1、Bax的表达情况与肝癌的临床病理、随访资料进行分析。结果肝癌组织中Mcl-1、Bax阳性率分别为60.4%(29/48)、31.3%(15/48),癌旁肝组织Mcl-1、Bax分别为14.5%(7/48)、52.1%(25/48),两者差异具有统计学意义(P<0.05)。肝癌组织中Mcl-1的表达与Edmonson grade分级、肿瘤直径、静脉浸润、肝内外转移有关(P<0.05)。在肝癌中Mcl-1与Bax的表达呈显著负相关(r=-0.373,P=0.009)。肝癌组织中III、IV级组Mcl-1/Bax≥1的比例显著高于I、II级组(P<0.05)。Mcl-1阳性病例的3年生存率显著高于Mcl-1阴性病例(P=0.032)。结论 Mcl-1在肝癌组织中呈高表达,与肝癌的分级、预后关系密切,Mcl-1、Bax在HCC的发生发展过程中起一定作用。
ObjeOive To study the clinical significance and relationship between the expression of Mcl-1, Baxin hepatocellular carcinoma and paracancerous tissue. To explore the correlation among expression of Mcl- 1, histologic grade and prognosis of hepatocellular carcinoma. Methods The expressions of Mcl-I and Bax were detected by EnvisionTM immunohistochemistry with 48 cases of HCC and paracancerous tissues. The circum- stance combined with clinical pathology and follow-up data were analyzed. Results The positive rates of Mcl-1 and Bax were 60.4% (29/48) and 31.3% (15/48) in HCC tissues, respectively. While that in paracancerous tissues were 14.5% (7/48) and 52.1% (25/48) respectively, with significant difference between them (P〈0.05). There was significant correlation among Mcl-1 expression, Edmonson stage, tumor diameter, vein invasion and intrahepatic metastasis (P〈0.05). Mcl-1 was negatively correlated with Bax expression in HCC tissues (r=-0.373, P=0.009). Mcl-1/Bax 〉 1 in hepatocarcinoma HI, IV grades were significantly higher than the that of I, lI grade (P〈0.05). The 3-year survival rate of Mcl-1-positive cases was significantly higher than that of Mcl-1-negative cases (P=0.032). Conclusion The high level of Mcl-1 expression has a close relationship with histologic grade and prognosis in hepatocellular carcinoma tissues, and Mcl-1, Bax both play an important role in the occurrence and the develop- ment process of HCC.
