摘要 目的探讨Scheuermann病的MRI表现特征及临床意义。方法回顾性分析2 164例腰椎基于较大视野扫描的MRI资料。结果 MRI诊断Scheuermann病23例,以30~49岁最多。该23例病程中有胸椎MRI扫描资料。均伴不同程度下背部疼痛、多个椎体楔形变、多发Schmorl结节及部分脊柱后凸畸形。Schmorl结节以T11~L4最多,MRI表现为椎体终板局限性缺损,椎间盘信号影疝入,周围可伴硬化边或不同程度椎间盘或终板变性,以矢状位显示最佳。腰椎MRI大视野扫描(矢状位扫描野40~43 cm)可一并将该病好发部位的下胸椎及腰椎带出,提高了该病的诊断率,该扫描方案性价比高。结论腰椎Scheuermann病具有较为特征性的MRI表现,基于大视野的腰椎MRI扫描对该症的诊断具有重要意义。 [Objective] To study the MRI features and clinical significance of lmnbar Seheuermann's disease. [Methods] A retrospective analysis of 2 164 cases data of lumbar MRI based on the large field of view. [Results] Lumbar Scheuermann's disease were found in 23 eases, mostly in 30 to 49 years old. 23 patients underwent thoracic MRI scan in the course of the disease. 23 eases were associated with different degrees pain of low back, multiple vertebral body wedging, multiple SchmorFs nodules, and some case with kyphosis. Schmorl's nodules were found mostly in T11-L4, MRI showing the intervertebral disk hernia into the defect of vertebral endplate, surrounding with hardened edges and different degrees of disc or endplate degeneration, and sagittal displaying the best. The images of the lumbar MRI scanning based on the large field of view (sagittal scan field with 40-43 cm) can display the disease predilection sites of lower thoracic, and lumbar simultaneously, so the diagnosis rate of the disease improved, and the scanning is cost-effective. [Conclusion] It is characteristic for lumbar Scheuermann's disease on MRI manifestation and it is of great sianificance to diagnose the disease with lumbar MRI scan based on the large field of view.
