曲唑酮治疗焦虑症

Trazodone in treatment of anxiety disorders

目的 :观察曲唑酮对焦虑症的疗效。方法 :选择符合CCMD 2 R诊断标准的焦虑症病例 90例(实际完成 87例 :男性 2 7例 ,女性 6 0例 ) ,随机分 3组 ,每组 30例 ,分别用曲唑酮 50～ 10 0mg·d- 1,po ,阿普唑仑 1.6mg·d- 1,po ,劳拉西泮 2mg·d- 1,po ,共观察 4wk。结果 :曲唑酮组的总有效率基本与其他 2组相仿 (P >0 .0 5) ,仅wk 4优于阿普唑仑组 (P <0 .0 5)。HAMA减分率与wk 1阿普唑仑组及各周劳拉西泮组差别无显著意义 (P >0 .0 5) ,但与wk 2 ,wk 4阿普唑仑组比较差别有显著和非常显著意义 (P <0 .0 5或P <0 .0 1)。药物副作用少而轻 ,以胃肠道不适和困倦感为主 ,仅 3例分别出现头痛伴焦虑、头晕伴烦躁不安、焦虑症状加重。结论 :曲唑酮是有效的抗焦虑药物 ,且副作用小。

AIM: To observe the effect of trazodone in the treatment of anxiety disorders. METHODS: Ninety outpatients (actually performed 87 patients: M 27, F 60) who met CCMD Ⅱ R criteria for anxiety were randomly assigned to trazodone group (50～100 mg·d -1 ), lorazepam group (2 mg·d -1 ), and alprazolam group (1.6 mg·d -1 ) for 4 wk, 30 for each group. Efficacy was assessed with the Hamilton Rating Scale for Anxiety (HAMA). RESULTS: The total effective rate of trazodone group was similar to that of other two groups ( P > 0.05 ), only better than that of alprazolam group in wk 4 ( P <0.05). The HAMA response rate (50% decrease from baseline) of trazotone was not higher than that of lorazepam and alprazolam groups in wk 1, but higher than that of alprazolam group in wk 2 and wk 4 ( P <0.05, P <0.01). The side effects were few and mild, and mainly showed unwell in gastroenteric tract and tired. Only three patients showed headache with anxiety, dizziness with restness, episode of anxiety got worse, respectively. CONCLUSION: Trazodone is effective and well tolerated for treating anxiety.
