鼻咽癌组织中浸润淋巴类细胞对瘤细胞凋亡的影响

EFFECT OF TUMOR INFILTRATING LYMPHOID CELLS ON NEOPLASTIC CELL APOPTOSIS IN NASOPHARYNGEAL CARCINOMA

目的 研究未经治疗的鼻咽癌组织中浸润淋巴细胞对癌细胞凋亡的影响。方法 中山医科大学所属病理科38例未经治疗的鼻咽癌活检组织采用末端标记细胞死亡检测法 (TUNEL ,Td T- mediated d U TP nick end labeling)计量每 5 ,0 0 0个癌细胞中凋亡的癌细胞数——癌细胞凋亡计数(Apoptosis Count,AC) ,采用 L SAB免疫组化法检测鼻咽癌组织中浸润的反应性 T淋巴细胞、B淋巴细胞、树突状细胞和单核 /巨噬细胞。各种淋巴类细胞指数以高倍视野下的细胞平均数表示 ,即反应性 T细胞指数 (T cell index,TCI) ,B细胞指数 (B cell index,BCI) ,树突状细胞指数 (dendriticcell index,DCI)和单核 /巨噬细胞指数 (Monocytes/macrophages index,MMI)。结果 (1)癌细胞凋亡计数(AC)在 38例未经治疗的鼻咽癌组织中波动较大 ,其均数为13.0 1± 13.2 1/ 5 ,0 0 0癌细胞。 (2 )癌细胞凋亡计数 AC与TCI呈正相关 ,与 BCI和 DCI不相关。 (3) AC与 MMI呈正相关。结论 (1)浸润于鼻咽癌组织中反映机体细胞免疫功能的反应性 T细胞 ,能够影响瘤细胞的凋亡 ;(2 )单核 /巨噬细胞指数 MMI与癌细胞凋亡计数 AC呈正相关的结果说明浸润的单核 /巨噬细胞也可影响癌细胞的凋亡 ,但也有可能部分癌细胞凋亡的后果 ;(3)与机体体液免疫功能有关?

Objective To investigate the effect of tumor infiltrating lymphoid cells on neoplastic cell apoptosis in untreated nasopharyngeal carcinomas (NPCs).Methods 38 nasopharyngeal biopsy samples from untreated NPC patients were collected. TUNEL (TdT mediated dUTP Nick End Labeling) method was used for quantifying the neoplastic cell apoptotic count (AC, number of apoptotic cells per 5000 neoplastic cells). LSAB immunohitochemistry was adopted to detect the reactive T cells, pan B cells, monocytes/macrophages and dendritic cells. The amount of various infiltrating lymphoid cells was also indicated (cell no. high power view) termed reactive T cell index (TCI), pan B cell index (BCI), monocyte/macrophage index (MMI), and dendritic cell index (DCI) respectively. Results (1)The AC was varied greatly from case to case in untreated NPSs, the mean value being 13.01±13.21/5,000.(2)The AC was positively correlated with TCI and MMI, but not with BCI nor DCI. Conclusion (1) The host cell immunity represented by tumor infiltrating reactive T lymphocytes is able to influence the NPC cell apoptosis. (2) A positive correlation between AC and MMI indicates that the infiltrating monocytes/macrophages might principallyt be an apoptosis inducer or might be a consequence of apoptotic debris. (3) The infiltrating B lymphocytes representing host humoural immunity and antigen presenting dendritic cells as well do not directly influence upon neoplastic cell apoptosis.
