摘要
目的通过对小细胞肺癌与非小细胞肺癌能谱CT增强扫描,探讨CT能谱成像定量分析在鉴别小细胞肺癌与非小细胞肺癌影的像学价值。方法回顾性选取病理已确诊的小细胞肺癌与非小细胞肺癌各20例,均用宝石能谱CT进行GSI扫描,在宝石能谱成像浏览器(GSI viewer)的软件上进行,得到能谱曲线含碘量图40~140keV能量水平的CT值及斜率值对比,最后进行数据分析。结果增强扫描中40~140keV能量水平的CT值中,40~70keV能量水平的CT值小细胞肺癌组低于非小细胞肺癌组,小细胞肺癌组与非小细胞肺癌组能谱曲线肿瘤内40~70keV兴趣区(ROI)碘含量斜率各为(2.07±0.76)、(3.08±1.16),两组组间斜率差异有统计学意义(t=3.23,P<0.05)。结论小细胞肺癌与非小细胞肺癌能谱成像上碘含量不同的特征,可以初步区分小细胞肺癌与非小细胞肺癌,为影像学鉴别诊断提供一定的参考价值。
Objective To explore the application value of quantitative analysis in distinguishing between small cell lung cancer and non-small cell lung cancer by the gem energy spectrum CT. Methods Twenty patients with small cell lung cancer and 20 patients with non-small cell lung cancer confirmed by pathology were collected. All the cases un- derwent GSI imaging of Gem energy spectrum CT, the CT values and slope value on 40-140 keV was got in gemstone spectral imaging viewer (GSI viewer) software. Then data was analyzed and compared. Results In the CT values on 40- 140 keV of gem energy spectrum CT enhancement imaging, the CT values on 40-70 keV in small cell lung cancer group was lower than that in non-small cell lung cancer group. Slope value within the tumor region of interest (ROI) on 40-70 keV was (2.07±0.76) vs (3.08±1.16), statistical significant difference of slope value was found between the two groups(t = 3.23, P 〈 0.05). Conclusion The characteristics of iodine content can initially distinguish between small cell lung cancer and non-small cell lung cancer in energy spectrum imaging, and it provide reference value for the differ- ential diagnosis in medical imageology.
