摘要
目的探讨参芪地黄汤治疗IgA肾病的可能作用机制。方法将60只SD大鼠随机分为正常组、模型组、雷公藤组、中药低剂量组、中药高剂量组,每组12只,采用"白蛋白+脂多糖+四氯化碳"联合法建立IgA肾病大鼠模型。中药低、高剂量组分别给予参芪地黄汤1.4、2.8g/100g灌胃,雷公藤组给予雷公藤多甙片0.625mg/100g灌胃,模型组、正常组给予等量生理盐水灌胃,每日1次。12周末处死大鼠,检测各组血肌酐、尿蛋白定量、血清白蛋白水平;免疫组化和Western-blot法检测肾组织nephrin、podocin蛋白表达;RT-PCR法检测肾组织nephrin mRNA、podocin mRNA表达。结果与正常组比较,其余各组尿蛋白定量均明显升高(P<0.05);与模型组比较,各给药组尿蛋白定量均明显下降(P<0.05)。与正常组比较,其余各组大鼠肾组织nephrin、podocin蛋白及mRNA表达均明显下降(P<0.05);与模型组比较,各给药组大鼠肾组织nephrin、podocin蛋白及mRNA表达均明显升高(P<0.05)。结论参芪地黄汤能有效降低IgA肾病大鼠尿蛋白定量,其机制可能是通过上调肾组织nephrin和podocin表达。
Objective To observe the effect and possible mechanism of Shenqi Dihuang Decoction [decoction of Dangshen (Radix Codonopsis ) , Huangqi (Radix Astragali ) and Dihuang (Radix Rehmanniae Recens ) ] on IgA nephropathy. Methods Sixty SD rats were randomized into normal group, model group, tripterygium group, low-dose Chinese medication group and high-dose Chinese medication group, with 12 in each. The IgA nephropathy rat models were established with combined method [-bovine serum albumin (BSA) + lipopolysaccharide (LPS) + CC14 ]. The low-dose Chinese medication group and high-dose Chinese medication group was given 1.4 g/100 g and 2.8 g/100 g Shenqi Dihuang Decoction intragastrically. The tripterygium group was given 0. 625 mg/100 g tripterygium wilfordii tablets intragastrically. The model group and normal group were given normal saline intragastrieally once daily. The rats were killed at the end of 12 weeks. The serum creatinine (Scr), urinary pro- tein excretions (UPEs) and serum albumin levels were detected. The expressions of Nephrin and Podocin in renal tissues were detected with immunohistochemistry and Western-blot method. The expressions of Nephrin mRNA and Podocin mRNA in renal tissues were detected with reverse transcription-polymerase chain reaction (RT-PCR). Results Comparing with the normal group, the UPEs in other groups were significantly increased (P〈0.05). Comparing with the model group, the UPEs in three administration groups were significantly decreased (P〈0.05). Comparing with the normal group, the expressions of Nephrin, Podocin, Nephrin mRNA and Podocin mRNA in renal tissues in other groups were significantly decreased (P〈0.05). Compa- ring with the model group, the expressions of Nephrin, Podoein, Nephrin mRNA and Podocin mRNA in renal tissues in three administration groups were significantly increased (P〈0. 05). Conelusion Shenqi Dihuang decoction can effectively decrease the urine protein of IgA nephrophaty rats, which may be relevant to up-regulating the expressions of Nephrin and Podocin in the kidney tissue.
