多指标联合评估急性心肌梗死患者PCI术后病情的研究

Prognosis evaluated by multiple indicators in the patients with acute myocardial infarction after percutaneous coronary intervention

目的联合监测急性心肌梗死（AMI）患者血清瘦素、脂联素、IL-6、TNF-α水平变化，研究其对AMI患者病情的评估和临床意义。方法采用酶联免疫吸附分析法（ELISA）检测20例健康对照组志愿者和20例急性心肌梗死（AMI）组患者的外周血清瘦素、脂联素、IL-6、TNF-α水平及其变化。并于6h内对AMI组患者溶栓，分别于24、48h后再次检测该组患者。结果AMI组血清瘦素、脂联素、IL-6、TNF-α水平分别为（17．41±2．98）g／L、（16．66±1．29）mg／L、（1509．17±106．31）μg／L、（23．11±2．99）g／L，显著高于健康对照组（P〈0．05），并且高于AMI组PCI后24h的血清瘦素、脂联素、IL-6、TNF-α[（12．83±3．51）g／L、（11．98±1．35）ms／L、（1315．28±103．27）μg／L、（19．01±3．10）g／L]水平和48h[（8．80±2．11）g／L、（8．71±1．25）mg／L、（1121．31±99．13）g／L、（9．33±2．19）g／L]水平（P〈0．05）。结论多指标联合监测对评估PCI术后AMI患者的病情变化和治疗效果有一定的参考意义。

Objective To evaluate clinical significance of combined monitoring of multipleindicators including serum leptin, adiponectin, IL - 6 and TNF -α in the patients with acute myocardial infarction （AMI） after percutaneous coronary intervention （PCI） . Methods The levels of serum leptin, adiponectin, IL -6 and TNF -α in 20 patients with AMI and 20 healthy volunteers as controls were evaluated by the enzyme - linked immunosorbent assay （ELISA）. The 20 AMI patients accepted PCI within 6 h, and the level of serum leptin, adiponectin, IL - 6 and TNF - α from each patient were monitored again at 24 and 48 h after PCI. Results The serum leptin, adiponectin, IL - 6 and TNF - α levels were increased in AMI group [ （ 17.41 ± 2.98）g/L, （ 16.66 ± 1.29 ） mg/L, （ 1509.17 ± 106.31 ） μg/L and （23.11 ± 2.99） g/L, P 〈 0.05 ] compared with control group and were decreased at 24 and 48 h after PCI in AMI group [（12.83 ±3.51） g/L,（11.98 ±1.35） mg/L,（1315.28 ±103.27） μg/ L,（19.01 ±3.10） g/L, P〈0.05] ; [ （8.80±2.11） g/L,（8.71 ±1.25） mg/L,（1121.31 99.13） μg/L, （9.33 ± 2.19） g/L, P 〈 0. 05 ]. Conclusion The measurement of serum multiple indicators are helpful to estimate the progress and the effectiveness of PCI treatment for AMI patients.
