超声在腺性膀胱炎鉴别诊断中的作用

The value of ultrasonography in differential diagnosis of cystitis glandularis

目的 :总结分析腺性膀胱炎二维和彩色超声共性和特点 ,探讨超声在腺性膀胱炎诊断与鉴别诊断中的作用。材料与方法 :对膀胱镜病理证实的腺性膀胱炎31例进行超声、膀胱镜及病理所见对比分析。结果 :本组声像图表现分为两型 :局限型6例 ;弥漫型25例。其共性和特点 :①病变局限于粘膜层 ,基底平坦 ,与膀胱壁强回声界线清晰 ;②基底部无肿瘤滋养动脉长入 ,无树枝状彩色血流及中高速度动脉频谱显示 ;③弥漫型并发上尿路梗阻 ,多呈双侧性 ,且为不完全性。结论 :腺性膀胱炎声像图共性和特点有别于恶性膀胱肿瘤 ,因此超声对本病与膀胱肿瘤的鉴别诊断有重要价值 ,可以作为腺性膀胱炎初诊的首选检查方法。

Objective:We analyzed the common and specific characteristics of two dimensional and color ultrasonography,to investigate the value of ultrasonography in diagnosis of cystitis glandularis.Materials and Methods:The manifestations of 31 cystitis glandularis cases diagnosed by pathology were compared by ultrasonography,cystoscope,and pathology.Results:There were two types of sonogram,6 cases were typical type,25 cases were diffused type,the common and specific characteristics were as follows:①The lesion was limited in the mucous layer,the fundus was smooth,with distinct limit from the bladder wall.②There were no feeding arteries in the fundus,arborescent blood flow,and medium to high velocity arterial frequency spectrum.③For the diffused type with upper urinary obstruction,it was often bilateral and incomplete.Conclusions:The sonogram characteristics of cystitis glandularis are different from those of bladder malignant tumor,so that ultrasonography has great value in distinguishing cystitis glandularis from bladder malignant tumor and it can be used as the first choice for cystitis glandularis screening.
