脑梗死患者溶栓前后的多项炎性因子及VEGF、sICAM-1、sVCAM-1、MMP的变化规律研究

Study on the variation of multiple inflammatory factors and VEGF,sICAM-1,sVCAM-1,MMP in patients with cerebral infarction before and after undergoing thrombolytic therapy

目的:探讨脑梗死患者溶栓前后的多项炎性因子及VEGF、sICAM-1、sVCAM-1、MMP的变化情况。方法:选取2010年9月～2012年10月于本院采用溶栓治疗进行干预的的37例脑梗死患者为观察组,另外选取同期的37名健康人员为对照组,然后将观察组溶栓前及溶栓治疗后1、3、7d及14d和对照组的血清炎性因子及VEGF、sICAM-1、sVCAM-1、MMP水平进行比较。结果:观察组溶栓前至溶栓治疗后1、3、7、14d的血清炎性因子及VEGF、sICAM-1、sVCAM-1呈现先升后降,而MMP呈现持续降低的趋势,而观察组溶栓前及溶栓治疗后1、3、7d的血清炎性因子及VEGF水平均高于对照组,溶栓前及溶栓治疗后1d的sICAM-1及sVCAM-1高于对照组,MMP则于治疗前至治疗后1、3、7d及14d均高于对照组,P均<0.05,均有显著性差异。结论:脑梗死患者溶栓前后的多项炎性因子及VEGF、sICAM-1、sVCAM-1、MMP的变化较大,可作为监测溶栓治疗效果的重要指标。

Objective： To study the change of multiple inflammatory factors and VEGF,sICAM-1, sVCAM-1,MMP of patients with cerebral infarction before and after undergoing thrombolytic therapy. Methods：A total of 37 patients with cerebral infarction who were treated with thrombolytic therapy in our hospital from September 2010 to October 2012 were selected as observation group,and 37 healthy personal at the same time were also enrolled as control group, and the serum inflammatory factors and VEGF, sICAM-1,sVCAM-1, MMP of observation group before and after undergoing thrombolytic therapy were compared with that of the control group at first,third, seventh and fourteenth day of treatment. Results： The serum inflammatory factors and VEGF, sICAM-1, sVCAM-1 of observation group before undergoing thrombolytic therapy and at first, third, seventh and fourteenth day of treatment increased first and de- creased afterward,MMP showed a decreasing trend,and serum inflammatory factors and VEGF of observa- tion group before undergoing thrombolytic therapy and at 1st,3rd and 7th day of treatment were all higher than those of control group, slCAM-1 and sVCAM-1 level before undergoing thrombolytic therapy and first day of treatment were higher than those of control group,MMP at all time points were significant higher than those indexes of control group （P all〈0.05）. Conclusion： The fluctuation of multiple inflammatory factors and VEGF,sICAM-1,sVCAM-1,MMP of patients with cerebral infarction before and after undergoing thrombolytie therapy is obviuos,and can be used as important indexes for monitoring the treatment effect.
