糖化血红蛋白水平对冠状动脉病变程度的预测价值 被引量:11

摘要 目的探讨糖化血红蛋白(HbA1C)水平对冠状动脉病变程度的预测价值。方法回顾性分析2009年2月~2011年10月行冠状动脉造影明确诊断为冠心病的患者2 336例。将其分为单纯冠心病组和冠心病伴糖尿病组。冠状动脉病变程度分为重度狭窄和非重度狭窄。冠脉按狭窄累及冠状动脉的支数分为单支病变、双支病变和多支病变。对两组间的一般临床资料及冠状动脉病变程度等相关数据进行统计学分析,并绘制受试者工作特征曲线(ROC)预测冠状动脉重度狭窄及多支病变。结果两组间HbA1C水平及冠脉病变程度差异均具有统计学意义(P<0.01)。多因素Logistic回归分析示年龄、性别、高血压、HbA1C、GLU、TC、HDLC是冠状动脉病变的危险因素。HbA1C预测重度狭窄的灵敏度为45.3%,特异度为79.2%,ROC下面积为0.581;预测三支病变的灵敏度为86.3%,特异度为94.3%,ROC下面积为0.941。结论 HbA1C水平是影响冠状动脉病变程度的危险因素,对预测冠状动脉多支病变有重要价值。
