腹腔镜胆囊切除手术前后肝外胆管直径变化的临床研究

The clinical research of the extrahepatic bile duct diameter changes before and after laparoscopic gallbladder excision

目的对胆囊切除手术前、后肝外胆管直径的变化情况进行研究分析。方法选取我院2008年6月～2010年5月期间住院治疗的胆囊手术切除患者的临床资料进行回顾性分析,其中胆囊结石患者35例、胆囊息肉患者29例,均行胆囊切除术,并分别于术前3个月、术后3、6个月应用B超测量患者的肝外胆管直径,准确记录各时间段数据,进行统计学分析。结果 64例患者术前、术后3个月及术后6个月测量的肝外胆管直径分别为(5.4±2.2)mm、(8.5±2.3)mm及(5.3±2.0)mm。其中35例胆结石患者术前3个月及术后3～6个月时其肝外胆管直径分别为(5.1±2.0)mm、(8.6±2.2)mm及(5.4±2.1)mm;29例胆囊息肉患者术前及术后3个月、6个月时肝外胆管直径分别为(5.6±2.3)mm、(8.3±2.7)mm及(5.1±2.0)mm。术后3个月肝外胆管直径增宽,术后6个月逐渐回至术前水平。结论胆囊切除术患者的肝外胆管直径数值,术后3个月均比术前有所增大,术后6个月时逐步回缩至正常水平。

Objective Research and analysis on the extrahepatic bile duct diameter changes before and after gallbladder surgery.Methods June 2008 to May 2010,46 patients clinical data were retrospectively analyzed,including cystic and calculous 35 patients,patients with gallbladder polyps is 29,all line cholecystectomy,and respectively on preoperative and postoperative 3,6 months application B ultrasonic measurement of patients with extrahepatic bile duct diameter,accurately record the time data,conduct statistics analysis.Results 64 patients with preoperative and postoperative 3 and 6 months after measurement of extrahepatic bile duct diameter were（5.4±2.2）mm,（8.5±2.3）mm and（5.3±2.0）mm.The 35 cases with gallstone preoperative 3 months and postoperative 3~6months,the extrahepatic bile duct diameter were（5.1±2.0）mm,（8.6±2.2）mm and（5.4±2.1）mm;29 cases of gallbladder polyp patients,the preoperative and postoperative 3 months,6 months the extrahepatic bile duct diameter were（5.6±2.3）mm,（8.3±2.7）mm and（5.1±2.0）mm.3 months after the extrahepatic bile duct diameter broadening,postoperative 6 months gradually return to preoperative levels.Conclusion Cholecystectomy patients with extrahepatic bile duct diameter value,three standard of postoperative than preoperative increase somewhat,postoperative 6 months gradually rebound to normal levels.
