摘要
目的探讨慢性肾脏病(CKD)患者血浆五聚素3(PTX3)水平与肾小球滤过率(GFR)、24 h尿蛋白定量的关系。方法将90例CKD患者分为CKD3期组(A组)、CKD4期组(B组)、CKD5期组(C组),每组30例,另选健康正常人15例为对照组。各组均常规检测尿素氮、肌酐、24 h尿蛋白定量等,用简化MDRD公式计算各组GFR,用放免法测定PTX3、高敏C反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)水平。结果 A、B、C 3组患者PTX3、hs-CRP、TNF-α水平均明显高于对照组;单变量分析显示,PTX3与CKD患者的GFR呈直线负相关(r=-0.69),PTX3与CKD患者的24 h尿蛋白定量呈直线正相关(r=0.46)。结论 CKD患者体内存在炎症状态,CKD患者的肾功能、尿蛋白与PTX3关系更为密切。
Objective It is to investigate the relations between plasma levels of Pentraxin 3 and glomerular filtration rate (GFR) , 24-hour proteinuria in patients with chronic kidney disease( CKD). Methods 90 patients with chronic kidney disease were divided into 3 groups : stage 3 CKD group ( group A) , stage 4 CKD group ( group B) and stage 5 CKD group ( group C) with 30 cases in each group, and 15 healthy people were chosen as control group. Plasma Pentraxin 3 (PTX3) , high-sensitive C- reactive protein (hs -CRP), Tumor necrosis factor-alpha (TNF-a), GFR, 24-hour proteinuria were studied. Results The levels of plasma PTX3, hs -CRP, TNF-a were significantly higher in chronic kidney disease than those in control group. Univariate analysis showed PTX3 was negatively correlated with GFR, and it was positively correlated with 24-hour proteinuria. Conclusion Chronic inflammation was existed in patients with chronic kidney disease, renal function, urinal protein of the patients with CKD were closely with plasma PTX3. Comparing with CRP ,Plasma PTX3 is more related to GFR and pro- teinuria . It increases as GFR delines and decreases when 24-hour proteinuria increases.
