重型颅脑损伤急诊手术的麻醉方法分析

Analysis of Anesthesia of Emergency Operation for Severe Craniocerebral Injury

目的分析重型颅脑损伤急诊手术麻醉方法,从而找出安全有效的麻醉方式。方法将2009年2月—2012年7月在我院进行重型颅脑损伤急诊手术的患者126例作为研究对象,将其随机分成对照组和观察组,各63例。所有患者选择同样的麻醉诱导用药,对照组使用丙泊酚静脉维持,观察组通过异氟烷吸入维持麻醉,然后对两组患者的临床资料进行对比。结果两组患者插管前后血流动力学指标比较,差异均无统计学意义(P>0.05);但是在病死率、致残率、并发症发生率方面观察组显著优于对照组,差异有统计学意义(P<0.05)。结论保持患者呼吸道通畅、抗休克治疗以及应用适当的麻醉方法是确保手术顺利完成、降低死亡和并发症出现的重要保障。

Objective To analysis of severe craniocerebral injury in emergency operation anesthesia method,so that the safe and effective anesthesia method.Methods In February 2009 to July 2012 in our hospital for emergency operation for severe craniocerebral injury patients for 126 cases as the object of study,they were randomly divided into the observation group and control group,each of 63 cases.All patients choose the same anesthetic induction treatment,while the control group using propofol intravenous,observation group by isoflurane inhalation anesthesia,and then on the two groups of patients with clinical data analysis.Results Comparison of hemodynamic indexes of two groups of patients before and after intubation,the difference is not statistically significant（P0.05）;but in the mortality,disability rate,complication rate of observation group was significantly better than the control group,the difference was statistically significant（P0.05）.Conclusion To keep the patients respiratory tract unobstructed,anti-shock treatment and application of appropriate anesthesia method is to ensure that the operation was successfully completed,reduce the mortality and complication of important guarantee.
