摘要
目的 :探讨颈部拔伸法的生物力学作用机制。方法 :用生物力学电测技术观察颈部拔伸法的应力分布。实验方式 :在体状态下实时描计拔伸法时下颌及乳突三点的受力情况。结果 :1.各颈椎横截面的拔伸应力分布近背侧为正应力、近腹侧为负应力 ,呈现前压后拉 ,最大应力分布在棘突及其周围。 2 .受试者头颅由自然位→前屈位 ,同一颈椎节段最大拔伸应力逐渐增大 ,并随前屈角度增大及颈椎节段下移 ,增大趋势更为明显。而对于整段颈椎 ,当前屈角度不变 ,最大应力随颈椎节段下移趋于逐渐增大 ;当拔伸力度相对衡定 ,头颅自然位时 ,最大应力随着颈椎节段下移其增大的趋势较为缓慢 ;当前屈角度增大 ,最大应力也存在增大趋势 ,但幅度大为增加。结论 :在小角度前屈时使用颈部拔伸法不但安全 ,而且可获得最佳效果。
objective:To probe the biomechanics approach of cervical intermittent pulling manipulation.Methods:The real time and strain-resistance measurement technique were applied to observe the stress distribution at the three point,which are jowl and two mastoids,on living human body during the manipulation.Results:①The traction stress distribution on transverse section of each cervical vertebra is that,ventral side is positive while the dorsal side negative.i.e.,anterior pressing but posterior pulling.The biggest stress is distributed around the spinous process.②With the head of the testee is flexed more and more,the biggest traction stress on every same vertebra becomes stronger and stronger.As the flexion angle becomes bigger and the section lower,this stress enhancement will be more obvious.For the whole cervical spine and fixed flexion angle,with the cervical section moved lower and lower,the biggest stress increased more and more.If the head kept in nature position and the pulling force in identity,the trend in which the biggest stress in creased with section moved lower is alleviative. Suppose the flexion angle is increased,the biggest stress is also has the trend of enhancement. Conclusion:The cervical intermittent pulling should be manipulated in mild flexion.Thus is not only safe,but most effective also.
