摘要
目的观察地佐辛在阑尾切除术硬膜外麻醉中的临床应用效果。方法选择2011年9月至2012年2月在成都市第五人民医院急诊行开腹阑尾切除术的患者40例,随机数字表法分为地佐辛组(A组)和对照组(B组),各20例,于切皮前5 min各静脉注射地佐辛0.1 mg/kg或等量的生理盐水,观察两组患者手术前、给药后5 min、牵拉阑尾时的心率、血压、呼吸频率和血氧饱和度,并观察恶心、呕吐、呼吸抑制、心率减慢等不良反应的发生情况。结果 A组比B组能更好地维持生命体征的平稳,A组能有效地抑制阑尾牵拉反射,并降低恶心、呕吐发生率(P<0.05),呼吸抑制和心动过缓A组1例,B组心动过缓8例(P<0.05)。结论地佐辛在阑尾切除术硬膜外麻醉中抑制阑尾牵拉反射的效果确切,呼吸、循环维持稳定,安全可靠。
Objective To observe the clinical effect the dezocine in epidural anesthesia appendectomy.Methods 40 cases of emergency appendectomy of ASA Ⅰ-Ⅱ were selected and randomly divided into two groups,group dezocine（group A,n=20）and the control group（group B,n=20）,5 minutes before operation,injection 0.1 mg.kg-1 dezocine was administered in group A,but in group B physiological saline of the same amount was given.HR,MAP,RR,SpO2 were observed and recorded before operation and 5 min after administration,and during dragging appendix.At the same time,nausea,emesia,respiratory depression,heart rate decrease and other adverse reactions were observed and recorded.Results Vital signs of patients in group A were more stable than group B（P0.05）.Intraoperative traction reaction and incidence of nausea and vomiting in group A was less than group B（P0.05）.There was 1 case of respiratory depression and decreased heart rate in group A and 8 cases of decreased heart rate in group B（P0.05）.Conclusion In epidural anesthesia appendectomy dezocine can suppress appendectomy stretch reflex and stabilize respiration and circulation with affirmative effect,which is safe and reliable.
