原裂孔气液交换治疗复杂性视网膜脱离的早期效果评价

The short-term study of treatment for complex retinal detachment with fluid-air exchange through the original retinal breaks

目的 评价玻璃体手术治疗非后极部裂孔复杂性视钢膜脱离采用原裂孔作气液交换术后的早期效果。方法 回顾分析了 5 1例 (5 1眼 )非后极部视网膜裂孔的复杂性视网膜脱离病例的临床资料 ,手术方法均采用环扎加压术加玻璃体切除术 ,利用原裂孔进行气液交换 (fluid- Air exchange,FA)后眼内填充 C3F8气体 ,其中 2 2眼行完全 FA术 ,2 9眼行不全 FA术。结果 全 FA组与不全 FA组术后第 1天眼内气体充满、始终未充满病例及第1、2天高眼压发病率比较有明显差异 (P <0 .0 5 ) ,第 2、3、7天眼内气量及高眼压发病率、1周内视网膜复位率无明显差异 (P >0 .0 5 )。结论 利用原裂孔进行气液交换治疗非后极部裂孔复杂性视网膜脱离可取得较好的早期效果。

Objective To evaluate the effect of fluid-air exchange through the original retinal breaks in the peripheral or equatorial region during vitrectomy for complex retinal detachment(RD).Methods Retrospective analysis the clinical data of 51 cases (51 eyes) of complex RD which were performed in scleral encircling and buckling and pars plana vitrectomy with fluid-air exchange through the original retinal breaks and intraocular C 3F 8 gas tamponade,in which 22 eyes were undergone complete FA and 29 eyes incomplete.Results In postoperative days,there were significant differences in gas-filled eyes on the 1st day and after a week and in the incidence of intraocular pressure(IOP) greater than 25mmHg on the 1st and 2nd days (P<0 05),while no significant differences in gas-filled eyes and the incidence of IOP(>25mmHg) on the 2nd,3rd and 7th days,and the incidence of retinal reattachment within the 1st week between the complete and incomplete FA group (P>0 05).Conclusion Fluid-air exchange through the original retinal breaks for treatment of complex retinal detachment is effective in the short term.
