晶状体超声粉碎技术在玻璃体视网膜手术中的应用

Application of lensultrasonic fragmentation for vitreoretinal surgery

目的 探讨超声粉碎技术在患有白内障的复杂性视网膜脱离和为切除前部 PVR必须切除晶状体的玻璃体切除手术的应用效果观察。方法 1995年 2月至 1998年 2月连续用晶状体超声粉碎技术联合玻璃体视网膜手术对 40例 40只眼复杂性视网膜脱离进行治疗。方法是先行巩膜环扎后 ,除颞下为玻切手术灌注切口外 ,上方的两个切口 ,左手为晶状体粉碎另一个灌注头置入晶状体内 ,此灌注头用 9号弯针头即可 ,右手用超声粉碎针投进入晶状体内 ,间断地粉碎晶状体同时吸引 ,保持在囊袋内操作 ,负压在 10 0～ 2 0 0 m m Hg,功率值设在 10～ 12均可。最后用切割头切除囊膜 ,再行玻璃体切除 ,巩膜外冷冻 ,气液交换后或眼内激光 ,再注入 16 % C2 F6 或硅油作为填充物。结果 经过 4～ 5 0个月的随访 ,平均为 14.7个月。有 32只眼得到解剖复位 ,成功率为 80 %。术后视力明显提高。主要并发症是术中有 3只眼晶状体核坠下玻璃体腔。结论 晶状体超声粉碎技术为患有白内障的复杂性视网膜脱离和为切除前部 PVR必须切除晶状体的玻璃体切除手术开辟了新的治疗途径。并对手术适应症、术中注意事项和术中并发症的处理进行了讨论。

Objective To approach the technique of lensultrasonic fragmentation used in complicated retinal detachment with cataract and lensectomy for APVR.Methods Forty patient (eyes) with vitrectomy and lensultrasonic fragmentation were reviewed retrospectively.Results After average flollow-up of 14.7 months,the retina was completely attached in 32 eyes,the reattachment rate was up to 80%,visual acuity achieved improvoment.The complication is lens nucleus of three eyes fell in vitreous cavity.Conclusion Lensultrasonic is new treating way for complicated retinal detachment with cataract and lensectomy for cutting APVR.It was discused to the indication surgical procedures in different groups.and management of the complication.
